Detailed Answer:

Hello!

Welcome and thank you for asking on HCM!

Regarding your concern, I would like to explain that resynchronization therapy (CRT) is usually effective at improving cardiac performance.

Seems that her systolic function has obviously increased over the course of several months (almost doubled).

Improvements in left ventricular structure (reverse ventricular remodelling) and function are expected to be present over the course of one year and even after (accoridng to the major clinical trials on CRT).

So, I believe that further improvements may be possible even after 8 months of the implantantion procedure.

If you could upload her cardiac ultrasound reports (before and after the procedure), I could give a more specific professional opinion.

Hope to have been helpful to you!

Feel free to ask me whenever you need!

Kind regards,

Dr. Iliri

My opinion as follows:

Detailed Answer:
Hi, dear XXXXXXX

I carefully reviewed your attached medical report and ECG, and would like to confirm that the CRT device is functioning perfectly well (optimal sensing and pacing atrial and ventricular conditions).

Also, there has not been registered any dangerous arrhythmic events.

From ECG recordings, paced rhythm is present with acceptable QRS duration. So, everything seems to be fine!

A cardiac ultrasound follow up is necessary to investigate directly the beneficial effects of resynchronization therapy on cardiac performance.

Best wishes,

Dr. Iliri

Brief Answer:
Periodically scheduled echos are necessary to evaluate further improvements

Detailed Answer:

Hi again, dear Hasna!

I reviewed your previous mother's cardiac ultrasound; it seems to have been a severely impaired left ventricular performance.

But from the other hand CRT has consistently improved ejection fraction.

And it will be shown by new follow ups echo, whether further beneficial effects (in reverse remodeling) will be present even one year after the implantation procedure.

Also her therapy seems to be appropriately fitted to her clinical conditions.

A careful medical follow up will be necessary to address the above issues over time.

Best regards,

Dr. Iliri
