How Could I Correct My Hormonal Levels And Get Back My Periods Uninduced?
Question: Hi, (28 F).. irregular periods since 2012 (worsened with time that no period in 6 months also), married in 2014, trying to conceive since married and found out to be PCOD in 2015. In concise, went through 5 cycles of IUI. For the first 2 cycles I had one good matured follicle on day 14 or 15 but my husband was low (first it was 8 mill/ml and next it was 3 post wash). For next 2 cycles (husband started on fertisure and spemen) with another doctor I had 2 follicles each cycle (3rd cycle it was 8 and 4this 60) but early at day 10. Then doctor suggested me to go through laparoscopy to find out if any minute reason that's stopping me from pregnancy (laparoscopy on May 2018 on day 10). Findings were tubes open, no cysts, performed HSG meanwhile and no abnormalities. Even though doctors expect period to be on its own, it was delayed. So had to use Meprate to induce period.  5th IUI (June) 2 good follicles (at day 15) with 44 mill/ml count, currently in 2 week wait. Uterine lining is good in all cases. I am 5.4 inches (ht.) and 60kgs (wt.). So my questions are: 1. Am in the right way of treatment? If so what's stopping me to get pregnant? 2. What is the reason for my PCOD? ( my mom has controlled diabetes past 8 years) 3.How could I correct my hormonal levels and get back my periods (un-induced)? 4. Even though everything is fine with IUI why it is not working for me? 5. what would be next best option other than IVF since we cannot afford it? 6. any other suggestions/investigations needed and also it would be helpful if you share approx cost involved for IVF (in mumbai/thane and hyderabad)?

Other than polycystic appearing ovaries no other abnormalities detected.
Next step should be an andrology check up for your husband to rule out any testicular function ,varicocele ,genital infection etc.
Need to analyze semen from a reputed lab to find out the motility and morphology.
42million count alone wont suffice .
After Seeing the detailed semen analysis I can help you further steps.
So get a fresh semen analysis with 3 days abstinence from a reputed lab before 12 30pm.

Hi,
Rapid Lenear motility is low and that could be the reason for not becoming pregnant.
How ever motility can be increased if any detectable causes like varicocele or infection. Hence I suggested an andrology check up for him.
In view of your PCOD and inadequate ovulation IVF may be a better option.
IUI with donor semen may also not help if the ovulation is not proper. Hence instead of waiting further you can either try 1 or 2 IUI from a different center or Go ahead with IVF.
