I Had Blood In Semen A Year Ago And Inflammation From Rectal Exam But No Nodules What Could Be These Symptoms?
Question: I had blood in semen a year ago it cured itself but came back out of nowhere a few days ago it has been an off and on problem for 3 years but I had a year of 0 blood in semen and I have no other symptoms whatsoever if I take antibiotics it causes worse problems for me including testicle soreness. No infection has shown up on urine screenings and I have 0 symptoms of prostitatus except blood in semen. I’m 42 and have just been frustrated and have no clue why this could be happening. I don’t have pain a urologist in pAst ruled me out for prostate cancer but no extensive testing were performed just indicated he noticed inflammation from rectal exam but no nodules or other concerns. Over time if I masterbate more frequently it tends to clear up over time. It’s a mystery.

It is understandable to be concerned about the recurring problem, in most persons, however, it is not associated with a serious medical condition. In some persons the exact cause is not found.
As you mentioned, it can sometimes be associated with conditions of the prostate such as infection or enlargement but there would usually be additional symptoms and the PSA would show changes.
Other causes are -(1) conditions of the urethra ( this is the tube in the penis through which urine and semen pass)
Infection, stricture (narrowing), cysts, polyps (benign growths)- can cause bleeding to occur
(2)seminal vesicles- these are glands that are involved in semen production.
Cysts in these glands can also cause this symptom
(3)there are systemic/generalized illnesses that are also associated with this condition eg high blood pressure, liver disease , bleeding disorder
Normally investigations would consist of a urinalysis and culture, PSA but these are the find infections and conditions of the prostate which you said have been ruled out.
Imaging with a transrectal ultrasound may be needed to take a look at it, it would be able to check the seminal vesicles along with other structures
Cystourethroscopy involves taking a look at the urethra
In many persons, as it did with you at one point, it resolves on its own.

Can it be hormonal? This only started at 40. Also how does it clear with continued masterbation over time only if it’s frequent? There is no pain. Is that indicating an injury or is it unexplainable. Is there any type of diet that can help prevent it from coming back? Not sure why it keeps coming back 6-9 months of no issues then returning is this common?

I tried increasing flushing by doing it 3 times in last 24 hrs it’s not much better. If it’s just bleeding in sperm with no other symptoms then it usually is not dangerous? I have normal urination and nothing else I did notice it came out after playing tennis one day could I have injured myself without knowing it? It’s just very alarming to see but I think it’s ok to wait a few weeks to see if it clears on its own? Usually it would clear over time it’s just a nagging issue so drinking more water does not help?
