Question: I've had this rash from head to toe for two weeks. It's gotten worse each day. I did experience intense itching and now it burns to even take a bath in just water. Plus my skin is feeling tight and starting to crust. I had an immediate care Dr treat me for hives on prednisone, benadryl and 2.5% hydrocortisone cream. I've taken all of this and it's only gotten worse. is this hives or something else?

Brief Answer:
Urticaria to be determine further

Detailed Answer:
Hello and thank you for asking in HCM

I am sorry for your situation.

I evaluate very carefully all your history you had provide with the previous doctors and i saw the pictures you had attached too.

Yes i agree that this might be urticaria according to the actual clinical picture

In my opinion and based in my experience in our hospital if we have a patient with your symptoms first step and the most important thing is to detect the causative agent (even in some cases it is not known)

Usually steroids and anti histaminics drugs work very well in this condition BUT it would be better that prior starting this treatment you would be better to do some tests for allergy and for autoimmune disorders that may be the cause of this hives:

Prick tests
Specific IgE
ANA XXXXXXX p XXXXXXX c
Hemogram
ESR
Urea and Creatinemia
CRP

Another think that may help too is the biopsy of the crusts .

Sometimes the cause is not found and we call Idiopathic Urticaria and the patient continues to be under treatment of steroids for several months.

The fact that for two weeks you are getting worse is an indication that the trigger factor is still present.

Please discuss with your doctor for your situation and for the above tests.

If you were my patient i would recommend the hospitalization for you just because of the fact you do all the necessary tests and examinations and to get more treatment according to the results (if it will be needed ).

Think about the above please

I wish a fast recovery for you

Take care
