I have gone through your question and understand your concern.
There may be possibility of disc prolapse at lumbar spine region with compression and canal stenosis leading to such symptoms.
Diabetes mat also lead to such symptoms.
You should get examined by a neurologist and get a MRI lumbar spine done.
Pain killers with muscle relaxants can be taken for short duration.
Pain modulator drugs like pregabalin or gabapentin can be added too.

As per attached reports and prescription, your balancing problem is due to spinocerebellar ataxia.
The back pain seems to be disc related and improvement is likely with medications and physiotherapy.

Presently I am taking dulot 30 which gives some relief in Diabetic nerve previously I was taking Pregabalin but it could not have some relief please advice the painkiller which I should take and the name of medicine Pregabalin and or gabapentin

You can increase the dose of duloxetine.
Gabapentin can be added if required.The dosage needs to be increased gradually for adequate response.
You can take tramadol or tapentadol for pain relief.
