Brief Answer:
Provide more details, communicate the treating neurosurgeon

Detailed Answer:
Hello,
I have gone through your question and understand your concern.
The images show a huge infarct involving right hemisphere and an infarct on left posterior region.
If you are not willing for surgery then communicate the treating neurosurgeon.
The present prognosis is not too good.
Please provide actual clinical condition of patient?
Is he on ventilatory support?
Any cardiac illness?
Medications like mannitol, 3% NaCl and diamox can be used along with other diuretics and antihypertensive.
Hope you found the answer helpful.
Do get back to me for further information.
