Question: Hi doctor

I recently engaged with nipple sucking with 3 massaeu .. none of them were lactating . I just sucked for may be 10 -15 sec briefly each time .. it was a tip suck gentle .. no did I bite them .. I didn’t notice any lesions also in their nipples nor discharge as far as I know .. no sexual intercourse , no oral sex , no handjob strictly only had a massage apart from this sucking of 10-15 sec each ... do nipple sucking cause hiv and what other STD likely to contract ? Do u recommend me a testing for hiv and what other things should I test for?

Thanks for posting your query.
Just nipple sucking in non-lactating women is absolutely a safer activity. You need not worry about any STD/HIV.
Even with lactating women who are HIV positive, sucking the breast once would unlikely be infectious. Only persistent sucking for weeks could be infectious as the infectivity through breast milk is very low when compared to blood, semen and vaginal fluid of an infected individual. If that individual is not having HIV infection that remote possibility is also not there.
If the individual is having an obvious breast ulcer due to STD, the this type of activity could cause an STD ulcer of an individual who sucks her breast, which is also quite unlikely. In your case you need not go for any test to rule out STD or HIV.

Dear doctor

Just a hypothetical question .. in case she has an ulcer and i didn’t notice .. would there be a possibility of transmission of hiv ?? What would be the likely STD which would transmit .. I m sure that were no ulcers or discharge .. just in case .. can I be 100% sure that HIV is not transmitted this way..coz I continuously went for massage for 4 days with brief sucking of 20-15 sec each day .. are there any remote possibility.. by a superficial contact of mouth with nipples ??

If there had been ulcer .. they might not have allowed me.. all of them were dry as far as I know without any scabs or ulceration .. moreover I never sucked them hard it was extremely brief as I limited them .. even if there any microscopic tears would that be a problem but I m 100% sure they were not lactating..

Ulcerative STD is very rare nowadays and unlikely to be missed.
If that woman had no HIV infection, you would not have any chance to get HIV even if she had ulcer over her breast. HIV woman with ulcer breast would likely to transmit the infection through breast suckling. But remote possibility only.
Except in breastfeeding infants, I am yet to see a case of HIV infection acquired through breast sucking either in my 28 years of my experience with HIV patients or in anywhere in the journals.

I hope I can move forward .. no need to test or worry .. I assume no one ever has contracted HIV this way .. do u recommend a testing after this instance ? Or shall I move forward and have sex with my wife without any hesitation.. I leave it to you doctor and i ll listen to wat u say

It was absolutely a breast sucking and it wasn’t that time bound may be 20 sec XXXXXXX each girl ..may be my tip of the tounge any little inner mouth was in contact .. nothing more .. no hard biting or extreme sucking .. it was mostly posterior. one extra activity is I did handjob myself not allowing them to touch my private parts .. I hope I can proceed with my life forgetting it

Time duration doesn't matter much. Your activities carry almost nil risk. You can continue your normal routine. If you have not convinced, have a safe sex till 12 weeks after the last contact with massage parlour girl, you can have a HIV test and rule out the least or remote possibility. Then you can resume unprotected sex with your wife.

I found one of the girl was lactating
She has a10 months old child
I asked her if she does check up she says she does every month because she has a child and she stopped feeding after 6 months
Since I made a sucking activity for 10 -15 seconds with her accidentally I got 2 drops in my mouth .. I believe there must be a risk in it .. in your opinion do u think it’s possible to get hiv this way ? I feel I must go for a test after 6 weeks

Breast milk is less efficient in transmitting the HIV in the presence of an infection. Breastfeeding for a few months could have the risk and not with a single episode. Moreover, as she had delivered a baby recently, she must have been screened for HIV and she would very much unlikely to have an infection.

unfortunately this happened in INDONESIA 5 days before on 09 August 2018 as I am onsite now .
Don't know the medical practice of checking of HIV before pregnancy
but she said the company who employs her sends for a regular check up every month
she said she stopped feeding her baby after 6 months.

I am sure these people are sex workers that they do for their family and they don't allow anyone without a condom

though I didn't suck her vigorously nor I felt any breast milk on my tongue .. it was very superficial with little tingling by tongue may be around less than 10 -15 sec ..
Read on the internet that saliva inhibits the infection

I believe all goes well .. considering the barring situations do u think any possibility of infection ?

I am sorry I am troubling you because my anxiety kills me even one drop is enough to infect if positioned properly

If I were in your position, and if I am a HIV specialist like this, and with the information I gathered so for, I will ignore, forgot and continue my day to day routine. If I don't have much knowledge about HIV, I will go for a test 12 weeks after the incident to get more clear cut idea and till then I will stick to safe sex.

Just for the reconfirmation I checked the massaeu again for lactation . She stopped feeding her at 6 months and I pressed and checked for lactation I couldn’t see and she said she is not lactating any more and also said she checks her blood once a month and she is free of hiv .. because she cares for her kid .. I think I must respect her words and I have to believe her and she confirmed she won’t have sex with clients .. I just like that asked if she is ready for sex and I give her money she strictly said no for that and it was a XXXXXXX for me .. I think I must trust her and move on .. what do u say doctor ? I believe your words that hiv is not spread this way .. I feel relieved after so much of stress .. thank u so much doctor for your this wonderful explanation n clarity you gave me ..
