Question: Hi. I was wondering id you could help me. I am 48 yr old and in premenapause. I am normal weight. No diabetes or cholestetol. Since last Nov 2019 i have been suffering a low tolerance to perfumes and aerosols and deodarants. I stay away from people smelling like fabric softener,cologne or other strong smells. I have always had a strong dislike of strong smells and can always smell much better than average person. However now i feel a bit of distress in normal situations too. I have completed allergy testing and its all normal except i have the highest grass and pollen allergy level 10. My main symptom is i feel a sense of dread around fragrance. Also i have very mild swelling in soft pallette. This mild swelling has become chronic and i wake up with it now, but it gets worse when i smell perfume, a dog, a tree or other chemicals. My allergey test showed negative IgE response to perfume,dogs or chemicals. I do not seem to have any other symptoms really except swollen mucosa in back of mouth possibly soft pallete and uvala area. Though when i look in mirror it is not noticable to naked eye. I take allergy tablets with mixed success. Throat spray seems to help a bit more as does a lozenge. This situation is primarily things I inahale. However i have notices that also vitamins and tablets that rub against backmof throat going down also cause the same mikd swelling. I have no swelling taking gel caps. Its more when they are abit chalky. Thank you

Thanks for posting your query on AAD.

Have read your above description in detail. From your description it is apparent that you are suffering from allergy. You may have had allergy(atopy) in the childhood also but in very mild or subclinical form and is being manifest now. There is a long list of substances to which a person can be allergic to, commonest being dust, smoke, dander of pet animals, strong perfumes or any other strong smells, food items, drugs to name a few.
All your symptoms suggest allergy. The IgE level may be normal at present cos your symtoms are mild. If you get it tested when your symptoms are more, the levels may come out higher.
Though your symptoms are mild at present but people with allergy are prone to develop serious manifestations esp since you have mentioned soft tissue swelling in your throat.
For the above reasons you need to take treatment for your symptoms . You should continue antiallergens. Acombination of monteleukast and an anti histamine like levocetrizine or fexofenatide to be taken once a day. Low dose steroids taken as inhalers will further help you. For a prescription and correct technique of taking inhalers, please contact your local PCP. You should avoid allergens to which you are allergic .If a smoker , quitting will help .

Thank you for reply. I went to an allergist and inhad the skin prick test and they tested me with my own perrumes that had caused mild throat swelling yet they did not cause any swelling during skin prick test. Also I simultaneiusly tested negative to IgE and am not allergic to m own dog, but other dogs with stronger odour. Wheni was youndg i did not have allergies other than hayfever at spring and grass.

Yes even with skin test its not possible to identity all allergens as only few common allergens are tested . Patients themselves learn to identity the substances they're allergic to over a period of time. As you have mentioned being allergic to grass and having hayfever in your childhood, it shows your predisposition to developing allergies to other things also. One may not know when and to what one may develop allergy but its good to be aware of all that allergy may lead to. If your symptoms are not troublesome, you may not take the treatment but the treatment suggested by me above is safe with minimal side effects. You will feel relieved off the nagging discomfort you may be having. If you decide to take corticosteroid inhaler, you must rinse your mouth with water after taking the puffs. This is to avoid any oral thrush. Your local PCP will also advise you accordingly.
