Hi. I Have Had A Lump On My Left Side

Question: Hi. I have had a lump on my left side neck come up in February. Placed on anabiotic’s and two shots of Roeseffern (spelling?) . The nurse practitioner said she could feel something in there and my lymph nodes were swollen. I went and got an ultrasound they said my thyroid gland was slightly enlarged and my lymph nodes were enlarged and they thought reactive however the ultrasound was eight weeks after the lump first came up. My white blood cell count was at 10 and then went down to seven which according to the range was normal. They then sent me to an ENT. He put a scope up my nose down to my vocal folds. He stated that my throat was angry and there was swollen lymph tissue in the back of my throat. I do not have tonsils but that area was also swollen. The ENT wanted to put me on medicine for stomach acid. I went back to the nurse practitioner she ran more bloodwork because of my weight loss and my general feeling horrible. The blood work came back with high amylase level and my BUN went down to four. She referred me to the gastroenterologist. The gastroenterologist took seven vials of blood( no results back yet). They did an ultrasound of my stomach and the only thing they told me was they couldn’t see my pancreas because of gas in my abdomen. I had a scope done and that showed severe erosive gastritis and they took biopsies. They also stretched the first 1/3 of my esophagus ( I was having problems with swallowing). I am waiting on the biopsy results. I am still not feeling well and still having problems swallowing, nothing tastes good food wise and I have vomited multiple times since my esophagus has been stretched due to nausea. Could you please give me your opinion on what’s going on medically with me

As per the details given by you firstly an ultrasound is not a test of choice to visualize pancreas and therefore a CT scan of the abdomen has to be done for the same.

A CT abdomen is the test of choice to detect pancreas related problems.

Now as they have stretched upper part of the esophagus the case again can be categorized as having an overlap of pathology between ENT and Gastroenterology. The swelling difficulty could be because of tonsillitis an infection of tonsils or it can also be a result of any pathology in the esophagus. An elevated TLC is suggestive of infection.

Vomiting is temporary and is very likely a result of inflammation as a result of putting the endoscope (Zofran 4mg thrice can be taken for the same and is over the counter) and over the counter anti-inflammatory like Tylenol could also be helpful in settling of inflammation.

Once biopsy reports are there, full-fledged treatment of gastritis would be started.
