Question: Hi Dr. Saghafi -- it's Missy! I thought I'd pose this as another query, to make sure you are afforded proper credit for all of the support you've given me over the last few weeks!

Hahaha on your take on our lovely political system, and I could not agree with you more! You won't be surprised at this one -- in the course of my Don Quixote attack on the windmill --er, I mean the cell tower -- I impressed upon our town board the fact that the notice requirements to adjacent property owners simply sucked and were insufficient. I had researched adjoining towns' code requirements, and all were much stricter, requiring certified mailings to adjacent property owners, and some required pictorial drawing of the impact of the proposed tower. So lo and behold, I receive a phone call from one of the town board members (who just happens to be running for town supervisor) who told me that my concerns didn't fall on deaf ears, and he wanted to sponsor a resolution to require certified mail notice to adjacent owners; he proceeded to ask me for the information on the other towns' codes lmao!!!! My response -- TOO LITTLE TOO LATE!!!!

I just hope that with my mode of thinking, I don't fall into the "post hoc, ergo propter hoc" fallacy of reasoning and start attributing any and all health malady to the drasted cell tower!!

As for the Missy update, I visited my neuro this past Monday, and Dr. D. -- YOU TOLD ME SO!!!!! She is a lovely and honest doctor and told me in no uncertain terms that I do not have ALS, nor do I have MS. She expressed exactly as you did, that the minor lesions and changes on my MRI are totally nonspecific and not worrisome at all. My EMG and nerve conduction were perfectly normal (I attached a copy of the report), with the small exception of some slower sensory conduction in my right ulnar nerve (with no corresponding clinical symptoms). So -- I am totally dismissing neuromuscular fears from my mind! If nothing else, this little neuro odyssey not only afforded me the opportunity to meet you, but inasmuch as I hated re-visiting the crazy "probable MS" diagnoses from 2005, I can now completely dismiss that.

On my nose front, as you know, the pathology report showed it was an exophytic schneiderian papilloma with no atypia, but I was still a bit on pins and needles, as I saw that the surgeon ordered further testing for high and low risk HPV and also for P16. Well,, you won't be surprised that yours truly got curious and had to google what P16 is -- I understand it's some protein that is "unleashed" to help stymie abnormal cell growth. Of course, I also ran across reference that complete absence of P16 could indicate a genetic mutation pre-disposing individuals to tumor growth in different areas of the body, so of course there went my crazy cycle of thoughts lol! In any event, today I received the report of those additional tests on the portal (which I have attached). Thankfully, the results showed it was LOW RISK HPV 6/11 and that the P16 was positive, so from what I can garner, my body is still capable of manufacturing that tumor suppressing protein lol! And kowing that the HPV etiology was the low risk type definitely put me at ease! In any event, I meet with the surgeon next Monday for my follow up, but I am glad to have gotten the results now.

As an aside, this was particularly scary for me, in light of the fact that our former XXXXXXX Bills quarterback from the early 1990's (remember when the Bills made it to the superbowl four years in a row and lost every time-- ugh?) - XXXXXXX XXXXXXX (who still resides in XXXXXXX and is active in wonderful charitable organizations here) has been battling oropharnygeal cancer for a few years now, presumably caused by the high risk HPV (as a matter of fact, my surgeon had also performed one of Jim's surgeries). The poor guy has had countless surgeries removing large portions of his jaw, reconstructive surgeries, radiation and chemo. So of course, his story is always big on the news around here.

So now I can add to my resume that I am also a "frustrated geneticist" after I've read about a dozen medical journal articles on P16 and it's implications in HPV related cancers. Hahaha -- I bet you're not surprised at that!!

I am so very appreciative of your thinking about me in the way you do by sending in questions and giving such complimentary ratings. I don't feel all of that is really deserved simply because I'm only interpreting things that are and have been normal and giving you what you already knew (from others) which are clean bills of health (albeit from an armchair 250 miles away!).... Glad that a colleague has echo'ed similar sentiments on your neurological exam and test results.

I am especially happy at the results of the EMG/NCV studies which showed NO abnormalities whatever on the needle exam...not even a few fasciculation potentials (which wouldn't be unheard of even in someone without a single complaint let alone one who has witnessed some fasciculation activity)....I guess that underscores how intact your neuromuscular junctions are and that there truly is nothing to worry about.

I'm also very happy to hear about your papilloma classification as being low risk as well as the fact you are good to go on your POSITIVE P16 protein expression which you have correctly interpreted as being a FAVORABLE biomarker for prognostication of future activity of the papilloma's course.

I feel compelled to provide you with Parma Neurology's telephone number JUST IN CASE you ever needed it in the future and wanted to just stroll out West for a minute (440.842.3816).

On the one hand I truly hope there's no need to see me for anything of a serious nature.
You are far too healthy to need my expertise. Anyways, it sounds as if you've got a very good Neuro doc in your own backyard! But of course, on the other hand I'm slightly ambivalent knowing you have now been shown to be so neurologically healthy that I may not have a chance to hear from you further...isn't that a DILEMMA of sorts? :(

I suppose I could continue Googling your activities to see how things pan out with that Cell Tower....Far be it from me to counsel an attorney on how to handle such matters but since you mention there seems to be at least 1 surrounding community who agrees with your perspective on reasonable notice to be given before such construction is approved or takes place...and how being so lax in your municipality's code of "reasonable" rules of engagement, even though they're not written (I'm sure there's a legal standard to meet in terms of doing what's just plain RIGHT for the community without having to spell it out in formal detail and punctuation)...well, the first thing that pops in my very lay legal mind is CLASS ACTION suit against the municipality and the Cell tower company for putting others at definite risk aside from those within the immediate proximity of the tower which in your case is literally your front yard!

Perhaps, that sort of concerted pressure would show the cell company that it may be less costly and give them less of a black eye publicly if they would simply come and dismantle their tower with the same precision and rapidity they put it up as opposed to having to litigate a Class action on top of any types of health concerns and complications that may come about in the next 2 decades since the kids in your community have at least that long to figure out the way to dismember the company if their parents' can't do it first.....I dunno....seems like a good Class action attorney could find a way to bring that tower down or at least get the company to put a reasonable amount of money aside in a fund that can be used in the event bad health outcomes start occurring....I think the latter is a long shot and simply admits guilt to something that everyone has fought for eons as being controversial if not baseless (so the companies say).

So sorry to hear about XXXXXXX XXXXXXX ...I recall him well playing with the Bills.....always used to daydream about him playing for the Browns but of course, nobody of quality ever wanted to squander their best career days in Cleveland! Let's see if we can get back to those Kardiac Kids days of yore! That is a tragic shame that Mr. XXXXXXX is suffering that way....I hadn't heard anything about his health on ESPN or other news sources....he's huge in the Football World.....I'm glad XXXXXXX is sharing their love with him....

Of course, I'm not surprised at your boning up on P16 by reading the journals....that's what good attorneys do, right? And now with my new found approach to advising folks on just how much they should read about things they THINK may be wrong with them....my recommendation to you is....become as expert as possible in whatever interests you about yourself or anyone for that matter. All I ask is that you go to good sources of information. Blogs sites are interesting and even entertaining but not very factual in some cases. I also don't put a whole lot of stock into Wikipedia....many things are lacking when it comes to the veracity of certain medical information and topics that I've noticed just in the field of neurology.

Knowledge is inherently good. I believe that to be a truism. Also, nothing bad comes from true knowledge except when such facts are tied together manipulatively for iniquitous gains without considering the greater good of the universe or the race. But again, it's not the fault of any of the information at hand that is the problem but rather the individual who either willfully or unwittingly uses the data to create negative impacts or Xi on person, place, or thing.

Are you guys getting those tornadic winds that we got all last night? Temps plummeted from 60 to 20 this morning. Yikes.....

Hello Dr. D! Yes, we were at the receiving end of those insane winds but much to my chagrin, they were not quite strong enough to take the cell phone tower down! Maybe some day, if I pray to the wind gods....

I would love more than anything to have a decent shot at a successful action in a court of law to bring that tower down! Unfortunately when I consulted with the attorney in NYC who specializes in that type of litigation, he indicated the probability of success once the tower is up drops dramatically from the chance of
success prior to the erection of these monstrosities. It’s so tremendously frustrating that whatever behind the scene machinations took place, the timing was such that we were “SOL” in having the ability to commence a special proceeding in state court to challenge the town’s approval. Of course it’s even more disconcerting to know that our elected officials don’t care one iota about protecting their constituents’ interests!

Thank you so much for offering your office number, and you most definitely have a standing invitation here in Western New York to come and visit!! Of course, you will be my physician of choice when I have my crazy and neurotic health worries and questions here on this forum, whether they be neuro related or not....you’d be thoroughly entertained with some of my “gems” (i.e. “my lord. where does this girl come up with these crazy questions??”) lmao!

Speaking of which, I knew I was fond of you for many reasons, but you referring to me as a “girl” at the ripe old age of 51 is certainly one of them!! I don’t know about you, but I feel as though I was just 21 like yesterday...! Yes, I’m the only female attorney at the office, amidst seven male attorneys. I started working there after my first year of law school as a law clerk, and am still there 29 years later!

Thank you for your input on the googling medical information, and I agree with you wholeheartedly in sticking to the reputable sites! The problem with me, however, is that although I can even understand what is in the medical journal articles for instance, I cannot
filter them to know what is and what is not applicable. And of course, believing it is all applicable to me is a recipe for disaster! On another note, I’ve even become totally disenchanted with Facebook and some of the articles that come through my news feed....some of these articles are positively incendiary in the medical realm! You won’t believe it, but last evening an article came up about some celebrity (I had never heard of him) who died of “nose cancer”! Yep....just what I needed to see after my nasal papilloma “ordeal” lol. Apparently it was squamous cell cancer in his nose that spread and metastized....I kid you not....the article showed a picture of him with his nose removed and him wearing a prosthetic nose! I couldn’t believe my eyes and had to show it to my husband! Then I stepped back to think more logically as to what the backstory was, as it said he has suffered from bleeding from his nose for a long time, etc. I would have been at the ENT within one week of having continuous bloody noses (and lord only knows what type of lifestyle this individual had)!! I’m just grateful I did not see that article until i was done with my nose issue....

Missy, Youthful/Energetic Lady of Many talents, with a successful academic career resulting in a successful entrance into a career/vocation that you've demonstrably dominated, in a work environment where it appears (at least by the standard of longevity) you are well appreciated and have been nurtured, as well as likely vice versa- successful family woman with progeny who are likely very close reflections to yourself and your husband, able to survive weather in XXXXXXX ...my...but that in itself--- how good it is to at least "imagine" your virtual voice reciting the above verses.... :)

Apologies once again for the lateness in response...perhaps, you can read this with some spare time between clients today and accept my apologies for being >24 hrs. late (the deadline that Healthcaremagic sets for docs to respond to queries)....I'm awful with deadlines, I must say. This weekend was full of preparation for the XXXXXXX XXXXXXX UNIVERSITY Wrestling Team's outing at its own JCU Open as the hosting school and so there were practices on Saturday with Setup in the afternoon and evening and then, yesterday of course, the event. If you've not surmised it at this point I am in a coaching role with the "kids"...(in my spare time)....volunteer Asst. When the tournament finished last evening and I helped roll, lift, drag, and store the mats (dressed in my usual BUSINESS ATTIRE that coaches typically wear) I had to go out to see some patients in the blizzard and poor visibility conditions out by the XXXXXXX XXXXXXX International Airport neighborhood....and by the time I got home in anticipation of being able to finally sit down and answer your interesting note.....I always like to grab a tall glass of Arizona Green Tea...and a few Jolly Ranchers to suck on as I read your notes.....but first sat on the couch! One of these days I have to start reviewing and practicing my parts for the musical I've consented to play in March, SOUTH PACIFIC....never done that particular one but I love it as I do every performing chance/GIG I can get my hands on...not only is the money enjoyable to receive...as a bonus to make ends meet which my violin teacher always said was the best decision I could've made professionally though I had received scholarships from several reputable conservatories in my youth......

I've done a bad job, I'm sure giving you a glimpse of my bio.....you can see why being a journalist/reporter would never suit my abilities....too much to say and it comes out my head in more of a ROUT than orderly ideas that any "reasonable" person to follow.....but somehow I think you're able to get me......Since I now believe I know which law offices you practice for....(great pic with the other attorneys BTW)....I definitely will give you a heads up when I know ther are firm plans to pass through your region of the country again....at least with the Wrestling Team if not to just come and visit the Falls! I'm very happy you did not run across all of the information and articles on the variations of a theme that things in the nose can transform themselves into since you may have been become unnecessarily worried about those items. I can't recall directly but didn't you explain to me in one of your previous notes some of the basis for your attitudes on health issues that can at times get magnified and become placed in the "wrong parts" of your brain? It had to do with your father's heart troubles, did it not? Excuse me if I'm not recalling things accurately at this point.

And I know you have a therapist that you are still engaged with.....but I'd almost like to know if you can objectively rate the received therapy you've gotten from this professional in terms of OVERALL VALUE to treating the underlying emotion or behavior which is probably a better term....do you think you've been helped 10%, 20%, 30, etc.? in terms of LICKING the problem. As an outsider looking in and making my own feeble attempts at judging the effects of this therapy on your situation, I think that there's definitely some room to say you've clearly improved from where things may have been some years ago when you were in your infancy of worries regarding YOUR OWN ISSUES....not so much your fathers'.....I say this because you seem to have now accepted (on the basis of your neurological clean XXXXXXX of health) the consensus that you do not possess any type of bad acting motor neuron disease nor MS!

If those 2 items have been wiped from your slate.....then, I believe you've made progress....Many people would not likely be able to really and truly put those topics to bed...no matter how many opinions and tests they obtain from good reputable professionals......but the tenor of your response from at least a time ago was pretty much...."off the list".....and that's terrific news.

In my mind, however, the real test of success in terms of being able to release you back into the Wild (ROFLMAO)....would be how can you expect (and what does your therapist expect) from the next health glitch that may be around the corner....since that's a given for everyone on the Planet....in a few years we will be talking about contradistinctions of people NOT on the Planet (HA!).....but how do you feel.....has your PARIETAL lobe wiring been sufficiently primed and secured for the next wave of sensory input that has to do with something regarding health concerns?

We've had this discussion before in small bits and pieces...but Missy, I would so love to feel that you are CURED.....there's very little doctors can actually CURE but if we could get you to avoid SCRATCHING the next ITCH and not feel a thing then, I feel we will have done you a world of UNIMAGINABLE nad UNQUANTIFIABLE good!

Missy, Youthful/Energetic Lady of Many talents, with a successful academic career resulting in a successful entrance into a career/vocation that you've demonstrably dominated, in a work environment where it appears (at least by the standard of longevity) you are well appreciated and have been nurtured, as well as likely vice versa- successful family woman with progeny who are likely very close reflections to yourself and your husband, able to survive weather in XXXXXXX ...my...but that in itself--- how good it is to at least "imagine" your virtual voice reciting the above verses.... :)

Apologies once again for the lateness in response...perhaps, you can read this with some spare time between clients today and accept my apologies for being >24 hrs. late (the deadline that Healthcaremagic sets for docs to respond to queries)....I'm awful with deadlines, I must say. This weekend was full of preparation for the XXXXXXX XXXXXXX UNIVERSITY Wrestling Team's outing at its own JCU Open as the hosting school and so there were practices on Saturday with Setup in the afternoon and evening and then, yesterday of course, the event. If you've not surmised it at this point I am in a coaching role with the "kids"...(in my spare time)....volunteer Asst. Coach was probably be the best designation though I was on the official coaching roster up to several years ago.....the new Blood coming in (some of whom I even coached over 10 years ago) overtaking the Head Coaching spot wanted to get a YOUNGER GENERATION in.....I resisted that unfortunate, unintended, and unavoidable slight to my value as a part of the team that I wrestled for when I but a lad some 35 years ago.....Mind you, I still am there to both put the hurt on some of these young whippersnappers as well as show how to TAKE the hurt when some of them make a go at COACH! But.....again, I digress!

Ergo, the lateness in the reply. When the tournament finished last evening and I helped roll, lift, drag, and store the mats (dressed in my usual BUSINESS ATTIRE that coaches typically wear) I had to go out to see some patients in the blizzard and poor visibility conditions out by the XXXXXXX XXXXXXX International Airport neighborhood....and by the time I got home in anticipation of being able to finally sit down and answer your interesting note.....I always like to grab a tall glass of Arizona Green Tea...and a few Jolly Ranchers to suck on as I read your notes.....but first sat on the couch! Hi Dr. Saghafi -- it's Missy! I thought I'd pose this as another query, to make sure you are afforded proper credit for all of the support you've given me over the last few weeks! Hahaha on your take on our lovely political system, and I could not agree with you more! You won't be surprised at this one -- in the course of my Don Quixote attack on the windmill --er, I mean the cell tower -- I impressed upon our town board the fact that the notice requirements to adjacent property owners simply sucked and were insufficient. I had researched adjoining towns' code requirements, and all were much stricter, requiring certified mailings to adjacent property owners, and some required pictorial drawing of the impact of the proposed tower. So lo and behold, I receive a phone call from one of the town board members (who just happens to be running for town supervisor) who told me that my concerns didn't fall on deaf ears, and he wanted to sponsor a resolution to require certified mail notice to adjacent owners; he proceeded to ask me for the information on the other towns' codes lmao!!!! My response -- TOO LITTLE TOO LATE!!!! I just hope that with my mode of thinking, I don't fall into the "post hoc, ergo propter hoc" fallacy of reasoning and start attributing any and all health malady to the drasted cell tower!! As for the Missy update, I visited my neuro this past Monday, and Dr. D. -- YOU TOLD ME SO!!!!! She is a lovely and honest doctor and told me in no uncertain terms that I do not have ALS, nor do I have MS. She expressed exactly as you did, that the minor lesions and changes on my MRI are totally nonspecific and not worrisome at all. My EMG and nerve conduction were perfectly normal (I attached a copy of the report), with the small exception of some slower sensory conduction in my right ulnar nerve (with no corresponding clinical symptoms). So -- I am totally dismissing neuromuscular fears from my mind! If nothing else, this little neuro odyssey not only afforded me the opportunity to meet you, but inasmuch as I hated re-visiting the crazy "probable MS" diagnoses from 2005, I can now completely dismiss that. On my nose front, as you know, the pathology report showed it was an exophytic schneiderian papilloma with no atypia, but I was still a bit on pins and needles, as I saw that the surgeon ordered further testing for high and low risk HPV and also for P16. Well,, you won't be surprised that yours truly got curious and had to google what P16 is -- I understand it's some protein that is "unleashed" to help stymie abnormal cell growth. Of course, I also ran across reference that complete absence of P16 could indicate a genetic mutation pre-disposing individuals to tumor growth in different areas of the body, so of course there went my crazy cycle of thoughts lol! In any event, today I received the report of those additional tests on the portal (which I have attached). Thankfully, the results showed it was LOW RISK HPV 6/11 and that the P16 was positive, so from what I can garner, my body is still capable of manufacturing that tumor suppressing protein lol! And kowing that the HPV etiology was the low risk type definitely put me at ease! In any event, I meet with the surgeon next Monday for my follow up, but I am glad to have gotten the results now. As an aside, this was particularly scary for me, in light of the fact that our former XXXXXXX Bills quarterback from the early 1990's (remember when the Bills made it to the superbowl four years in a row and lost every time-- ugh?) - XXXXXXX XXXXXXX (who still resides in XXXXXXX and is active in wonderful charitable organizations here) has been battling oropharnygeal cancer for a few years now, presumably caused by the high risk HPV (as a matter of fact, my surgeon had also performed one of Jim's surgeries). The poor guy has had countless surgeries removing large portions of his jaw, reconstructive surgeries, radiation and chemo. So of course, his story is always big on the news around here. So now I can add to my resume that I am also a "frustrated geneticist" after I've read about a dozen medical journal articles on P16 and it's implications in HPV related cancers. Hahaha -- I bet you're not surprised at that!! I hope you've been having a wonderful and enjoyable week! The weather has moderated a bit here, so it has been great to get rid of some of the snow from the blizzard! Missy :)
