Can Worm In The Stomach Lead To Itching Around The Anus?

Question: Hi Doctor,

My son is 3 years and 6 months old.
his weight is around 14kg
He is otherwise healthy.
About couple of months ago he used to get diarrhea and tummy pain(colic like pain) whenever he eats sweets such as chocolates, gummies, chewing gums and etc but it wasn't very serious. We never allowed to eat sweets until he was 2 and half years old.

about two days ago he had 7 sweets and on same day he developed painful colic pain (the pain starts suddenly and stays for about 1 to 3 minutes and goes away after a while and comes back again every couple minutes or hours). i started giving him some massage and he had some gas which i believe relieved him for a bit but it came back again.
Today the colic pain got worse almos half of the whole day he was crying from the pain and was playing when he felt reliefed and complains again, he was constipated for couple of days but he had normal soft stool yesterday but the pain didn't go away.
He also complained about right lower back pain since yesterday.

He doesn't have painful urination but he did urinate more than usual, even though i would give that reason to be that he was drinking alot of juice.
He also complained about itching on the anus.
no sign of appendicits when i palpate the right lower abdomen.the colic pain doesn't wake him at night.
He has slight fever of 37.5 degree celcius.

Questions.

1. does this seem like urinary infection given the frequent urination.or kidney infection given the pain he is feeling on the right lower back even though when i palpate the kidneys he doens't scream out of pain.
2.could it be worms given the itchy anus? we have a kitten in the house that's not dewormed but i don't recall him playing with the cats litter box or the cat moving it's stool to other surfaces of the house?
3. Generally what is the cause of Colic pain for a toddler at his age? i know babies do get colic pain but he is 3 years old?
4.could it be far more serious disorder such as ulcerative colitis or crohns disease?
5.what do you diagnose from this and what do you suggest i do?

Hello,

I can understand your concern. As per your history I think it may lactose intolerance. Let me know whether he has blood in stool? Kindly follow up with detailed history. So try to avoid dairy products as it contains lactose. Other causes stomach infection, Indigestion and gas. Kindly follow up with detailed history. I don't think this will be serious condition. Treatment mainly depends upon the underlying condition.

Common causes of frequent urination at this age are urinary tract infection, cystitis, drinking lots of fluid and rarely bladder stone, renal stone. Let me know whether he has burning micturation, blood in urine, high grade fever? Kindly follow up with detailed history.
You can do urine routine and culture sensitivity test, ultrasound abdomen and pelvis to rule out the other possibilities.

For itchy anus causes may be worm infestation, improper hygiene, sometimes allergies. You can give syp albendazole 10 ml if not given in last 6 months. Keep private parts clean. As these are prescriptions medicine so kindly concern with your doctor. I think this will clarify your concern.

Hi doctor,

thanks for the quick response, could you just clarify the reasons you suggest it's lactose intolerance? Interesting enough his mother had similar problems since she as his age and they couldn't find anything, she got better few years ago when she stopped eating/drinking diary food, so yeah the link could be there. but he usually has chocolate milk almost every morning and these symptoms don't happen except some times.

Today he is better with the cramping and colic pain, however he had frequent urination, no fever, painful urination or any discharge from the penis, no blood in urine, urine color is relatively normal.
His stool is normal slightly soft with no visible mucus or blood.

could you suggest for syp albendazole 10 ml , is it OD, BID or TID?
also do you recommend giving him probiotics?

Hello and welcome back.

Sorry for late reply. I can understand your concern. As in milk, lactose sugar is present. In lactose intolerance, he has deficiency of lactase enzymes required to digest lactose. So he is unable to digest it dairy products. You can confirm it by lactose tolerance test and stool acidity test. You can buy lactose free products or can add lactase enzymes tablets or drops i. e. Dairy ease before consumption. As these are prescriptions medicine so kindly consult with your doctor. Give alternative calcium rich products.

You can give probiotic which presents in some yogurt and also available as capsule form which is helpful to reduce symptoms.

As he does not have any symptoms associated with urination. So nothing to worry.

You can give syp. Albendazole 10 ml (400 mg) single dose at night if not given in last 6 months.

I think this will clarify your concern.
