Is Ksharasutra Procedure Painful While Treating Fistula In Ano?

Question: Hello doctor I have been diagnosed with fistula in ano. I am advised for a surgery. I am not interested in surgery so I heard about kshar XXXXXXX in ayurveda. Please let me know if it is a pain full procedure and how long does it take to get this done and side effects of it

Dear
Thanks for contacting with the Ayurveda section of [A]YUSH and after reviewing your health concern I'm of the following opinion:

i. Since you are a diagnosed case of fistula thus remain under the regular supervision of your attending Physician who is in a better position to explain the prognosis.

PS. any history of Sexually transmitted infections, Inflammatory bowel disease, Diverticular disease, or Rectal cancer?

ii. Anal fistula means there is an open track [which is communicating from exterior to interior], as a result, healing won't be there despite any conservative line of treatment.

iii. If surgical intervention is not done it will/may lead to multiple openings [fistula tracks].

iv. make sure after surgery the excised tissue is sent for histopathological examination [for the cause of infection] because post-operatively you will require antibiotics and examination will reveal whether the infection is general [simple antibiotics] or it is tubercular in origin [ATT regime] will follow.

PS. CBC [complete blood picture] and Routine Laboratory Investigations are mandatory, till then you can start incorporating following dietary regime and management:

-take fresh fruits and vegetables, avoid milk and milk products in the diet, an excess of tea, coffee, tobacco, alcohol and carbonated beverages.
- eat slowly, avoid haste-irregular eating and sweet/heavy food which are difficult to digest and cause indigestion.
- take less of strong spices (ginger, garlic), oily-fried-fatty food and flatus producing foods (peas, beans, dals)
- take old rice, bottle gourd, moong dal [green gram], XXXXXXX Marich [black pepper], pomegranate, XXXXXXX hot water, adequate rest and sleep.

- take fried cumin seeds 500mg,
fried asafetida (hing) 125mg,
XXXXXXX parpati 500mg...mix them and take with honey three times a day.

- 1 glass lukewarm water every morning and night.
- take elephant yam, papaya, buttermilk, pistachio, almonds, pear, old brown rice, milk, crystallized sugar, raw radish and avoid tea, coffee, garlic, onion, fish, hot spices, sitting for long hours.

PS: if job demands sitting, make sure you get up every half an hour to relax for 5 minutes and then sit back, make a sponge cushion for your chair.

vi. Kshara XXXXXXX [medicated alkaline thread] is an Ayurveda parasurgical technique to cure disease related to the Anal system.

PS.

i. As it is a surgical procedure thus small/minimal amount of pain is thereafter postoperatively which can be alleviated with the help of medicines and Anal hygiene.

ii. The duration of treatment in your case will depend upon the length of the tract, e.g. the unit cutting rate of Ksharsutra is about 1/2 to 1 cm per week so if the tract is 6 cm long, it will take 10 weeks to heal and usually 30 - 45 minutes are required to perform the surgery [remember that you may require several sitting, as already discussed it depend upon the type and extent of the disease]
