Question: Hello !

Than you for our previous answer !

As you are doctor you better understand autoimmune disease !

The qestios is
1.      Does chronic reactive arhritis with spondyloarthropathies can cause influmation bowel disease ? As i understand yes – and it ias about 5 – 10 % cases

Endoscopic lesions were more frequent in patients with spondylarthropathy (44%) compared with those with other inflammatory arthritides (6%

Conclusion. Gut inflammation is frequent in patients with spondylarthropathy, and one-fourth of the patients who have chronic disease have early features of Crohn’s disease
https://sci-hub.tw/10.1002/art.0000

between 5% and 10% of cases of AS are associated with IBD. Despite these observations, systematic screening of AS patients by ileo-colonoscopy is not indicated in the absence of gut symptomatology as only a small proportion of AS patients with subclinical gut inflammation will develop IBD in the future
https://www.ncbi.nlm.nih.gov/pmc/articles/PMC0000/#B5

2.     Spondyloarthropathy group : ankylosing spondylitis, undifferentiated spondyloarthritis, reactive arthritis, psoriatic arthritis, spondyloarthritis associated with IBD, juvenile onset spondyloarthritis

Does these disease like reactive arthritis, psoriatic arthritis …. Who are classified as seronegative spondylarthropathies are the same severe and with same symptoms and outcome as ankylosing spondylitis ?

It is better to understand - if you do your mind is more calm

Than you very much

Brief Answer:
hi as below

Detailed Answer:
hi
yes spondyloarthropathies are frequently associated with bowel pathology, even in those people who dont have inflammatory bowel disease, a high percent of people may have bowel pathology with ulcerations.
psoriatic arthritis sometimes presents with spondyloarthritis, back pain, however in other cases they may have a picture like rheumatoid arthritis with involvemnt of small joints.
These diseases like reactive , psoriatic may have similar features like ankylosing spondylitis but may not be as severe as AS, and may have some differences like asymmetrical arthritis, more involvement of cervical spine as opposed to lower spine.
they will all form a group of diseases called spondyloarthropathies.

Thak you for your amswer !

Can you please in short answer by pionts ! Then i better understand answers !!

Thank you very much

1.This groupe like like reactive , psoriatic arhritis ......that is in spondyloarthropathies groupe also cause 5 - 10 % case chronic influmation bowel disease ?

2.And parasitic infection very rare cause reactive arhritis ?

Parasitic infections

Parasitic infections of the gut due to Strongyloides stercoralis, Taenia saginata, Endolimax nana and Dracunculus medinensis have been associated with a form of reactive arthritis[36].

https://www.ncbi.nlm.nih.gov/pmc/articles/PMC0000/#!po=50.0000

3.

Parasitic Rheumatism among Patients Infected with Intestinal Parasites?

Does it go away after treatmen ?

Like this case

Results: The results showed that 50 patients out of 107 had parasitic infection. Cryptosporidium infection was the commonest
(48.0%) followed by C. cayetanensis (32%), G. lamblia (24%), B. hominis (20%) and E. histolytica (8%). Beside Microsporidia, S. man- XXXXXXX A. lumbricoides and S. stercoralis were 4% for each. Some patients had mixed and triple parasitic infection. Infected parasitic
patients fulfilling parasitic rheumatism criteria were 16/50 (32%). Giardiasis patients having parasitic rheumatism constituted 8/16
(50%) and were the commonest. The other eight (50%) patients had Cryptosporidium, C. cayetanensis, E. histolytica/dispar and S.
stercoralis. Two patients per each parasite. Giardiasis patients with parasitic rheumatism were females. Half of them (50%) were <
15 years, complained of mono-articular joint pain, while the other were adults, complained of poly-articular joint pain.
Conclusion: Due to similarity of musculoskeletal presentations of parasitic diseases to many rheumatic diseases; consequently the
importance of parasitic infection as an underlying cause of rheumatic syndromes especially in developing countries must be studied
in patients undergoing evaluation for rheumatic complaints.

Detailed Answer:
hi
1. yes this group is spondyloarthropathy, with a small group having predominant bowel symptoms related to inflammatory bowel disease.
2. yes these parasites are very rare causes more common are pathogens like shigella, e coli etc.
3 yes this should, go away with treatment however in those people who are predisposed to arthritis like a person who is hla b27 positive may sometimes tend to have prolonged arthritis related to this disease.

Thank you very much !

But stil i have some questions

1. The group of spondyloarthropathy like

Reactive arthritis-
Psoriatic arthritis-
anterior uveitis -
inflammatory bowel disease-

these groupe it is not severe as AS ?

Does some of these group are more sever as other like reactive arthritis spondyloarthropathy is more sever as Psoriatic arthritis spondyloarthropathy ?

2. How common is Psoriatic arthritis spondyloarthropathy in psoriatic arhritis patients ?

As in literature it is said - that 50 % can have Spondylitis, but it is together with other forms of psoriatic arhritis ! But only spondylitis affect only 5 % of all case !

The question is how much who is psoriasis arthritis spondyloarthropathy - that is like AS the group of 50 % or this small group of 5 % ?

Spondylitis
PsA can also affect the joints in the back
(called axial skeleton). The joints of the back
(spine) are involved in about half of people with
PsA. Inflammatory arthritis of the back
or neck

Spondylitis with
or without sacroiliitis
This pattern occurs in about 1 in 20 cases.
‘Spondylitis’ means inflammation of the joints
of the spine. ‘Sacroiliitis’ means inflammation
of the joint between the lower spine (sacrum)
and the pelvis. Back pain is the main symptom

Brief Answer:
hi

Detailed Answer:
hi
1.I wouldn't say that these categories are not that severe, yes maybe as far as the spine is concerned AS would be the most severe, however anterior uveitis, psoriatic arthritis and inflammatory bowel disease may be associated with severe eye issues/peripheral arthritis/ skin plaques/bowel related problems. so the problems may be manifold and extra articular problems may be as bad.
2.hi yes, spondyloarthropathy may not be the predominant manifestation, only a small percentage may be purely spondyloarthritis, in others it may be a silent manifestation along with the peripheral arthritis.
3 yes definitely with a great amount of research in this area, eye problems can be successfully treated with medications and blindness can be prevented
