What All Precautions Should One Follow While Recovering From A Cauterized Internal Bleeding Tear?

Question: Hello Dr. T.Chandrakant,
I apologize for not getting any reply lately.
The following was the message from the performing doctor:
The colon biopsy was benign. No cancer or pre-cancerous polyp was seen.
given the findings with the blood vessels in your stomach.I would like to follow up
your blood counts to see if there is improvement. lets plan to repeat your lab test in
April so to ensure your blood counts are coming back up. I have ordered the test already.
Try to go around the 2nd week of April.
I do not have the copy of the other messages, but i shall email the rest. I am not too ready
today.
Any way thank you so much for your concern.
There were a few things that distracted us this past weeks, especially my wife was called
for jury duty, and i made some mistakes in responding to it, hence it took too much of our
time. It was her first call for duty and things or procedures had changed. My wife was excused
just yesterday.

Once again, Dr. Chandrakant,

After the colonoscopy/endoscopy procedures, the Doctor told us that he cauterized a few small vessels
in her stomach which somehow leak some blood. He added that he might schedule another endoscopy
in two months. He prescribed Omeprazole.

In the meantime, here's his follow up report:
the omeprazole you only need to take for 2 months- I want the area to heal up from my therapy. I would
like you to be on some sort of acid suppression there after though-but perhaps you can use Pepcid 40 mg
twice daily thereafter. The reason for this is because I suspect that the small vessels in your stomach are
being injured by the acid production, so I want to decrease this is as much as possible over time. Pepcid is
safe to use long term.

The result of the blood test will follow. I have to copy the text again.

This next one is our doctor's reply to our message regarding the lab test for blood count.

Thank you for your message -i did see your latest labs-the anemia appears to be
improving as we hoped. I'll plan to have your primary care physician follow this count.
If you start having any dark stools let me know. We can repeat your procedure to
cauterize more.

If you have any questions please do not hesitate to contact us.

As for our message to him, I shall send another email, to complete the sequence of
events.

Very nice to know that the cause of bleeding is found and cauterized, improving the blood picture.
The advised given by your Doctor is perfectly okay and should help her to have proper recovery and possibly no recurrence.
She has to be watchful for the color of stool and it is worth getting the stool tested for blood/ occult blood when in doubt so that review endoscopy can be done earlier if needed.
Avoid all the foods and beverages that instigate or enhance the problems like oily and spicy foods, aerated drinks.
Control of stress and anxiety is also a part of the management.
Small frequent meals of soft bland diet help.
Continue the medical management for the problems as before.

Feel free to ask for further relevant queries if you feel that there is a gap of communication.

Good Morning, Dr. T Chandrakant,

We appreciate very much your input, as it calms or gives us some peace of mind.
You mentioned about the dark stool, and we are not sure if we overlooked to tell the doctor about it. Since my wife was prescribed to take Iron tablets twice a day, we felt that it automatically make the discharge dark.

We noted that before procedure, there was a postponement of the regular medicine including the Iron, vitamin C and supplement like Citracal. After the procedure, my wife was back on her regular meals and medications except the Iron tabs. She stopped the Iron, when the result of the lab work showed improvement. Her stool is normal color at that time. However, she was advised later that she has to continue with Iron tabs, hence the usual dark shade of stool appeared, again.

We have not mentioned this result to the performing physician yet, hence do we have to now? Our reason mainly is to wait for the next lab work this May to find out if the blood count has further improved. Are we okay with our decision to wait?

Thanks for your feedback and review queries.
Iron preparations can cause false positive occult blood tests and hence stoppage of Iron preparation for 3 - 5 days and then doing this test may help to know whether the dark or black color is due to bleeding or iron preparation. Pathologists doing this test can help more about the specificity about the tests.
Iron or blood in GI tract causes the stool black and is called malena.
Yes, I think you should tell your Doctor about this and ask for further advise.
Yes, you can wait for the blood tests.

Regular tests are the only way to ascertain, hence try to do all the required tests as advised; earlier if the stool is black in spite of holding Iron preparations, say for 5 days.
