Question: Hello Doctors, i need some advice... i always feel like having lack of sleep even though i slept for more than 10 hours or tried sleeping only six hours but result was same .i will have very deep sleep my friends noticed because i snore so much.. i always feel pain in my forehead its nearly almost full head but more pain in front side i am suffering this since 3 to 4 months if stress is the cause i even tried for not working for more then 3 days but its same...and sometimes early morning i feel like shivering from.my teeth... i will not have meals at correct time sometimes early or late . earlier i was feeling pain in my chest and doctors advised me from same app to go to walk after meal every night now its gone... please advice what steps do i need to take...i spend most of the time with laptop and XXXXXXX screens .i will be away from XXXXXXX only while sleeping..

We will try to help you in best way possible. First of all it's sad to know what you had been going through lately.

Based on the history you have shared, excessive somnolence with snoring and other symptoms as you discussed can be due to obstructive sleep apnea and needs evaluation with sleep studies and work up to rule out carbon dioxide retention with arterial blood gases.

Though obesity is one cause of sleep apnea any other structural cause in the oropharyngeal passage which may block air passage during sleep can cause the same. Also with such symptoms, endocrine workup especially Thyroid functions, sugar levels, and cortisol levels may also be needed as the derangement in any of these may also present with the same symptoms.

If these all come back normal and detailed physical examination by your primary care physician is unremarkable, then a psychiatrist review for workup of anxiety related issues seeing to overwork or improper working schedule needs to be workup and sleep and stress management with both pharmacological and non-pharmacological measure should be done.
