Pain in anus for 1 week, now the skin around it is red, pain is increased and she found some extra skin inside.
Not willing for surgery.

- Bed rest.
- Full diet with high fibers and plenty of oral fluids.

I can understand , yet better try to be seen by a Medical professional so that the diagnosis is done properly.

You may please give her any medicines which she has taken in past like Ibuprofen or Diclofenac whichever suits her.
Give after food or with milk. Also give her Pantoprazole or Ranitidine to avoid gastric acidity.

Try to push the mass inside after sitz bath, if the mass goes in, the pain may dramatically reduce.
