Question: Hello Doctor,

My mother 46 years old complaining lightheadness from 15 days, but from past 2 days head spinning and eyes rotating feeling which is short lived and no faiting happend.

Previous Medical History:

back pain, neck pain and pain on back side of head ( streching like feeling) 3 months back after consulting doctor in %E2%80%98Ask a docor%E2%80%99 concluded it as cervico- spondolysis and suggested the medicine, she is not facing back pain now but having light pain on neck and back side of head.

Can you please suggest what to do now?

I have reviewed your query and here is my advice.

From your description she seems to have spinning sensation due to cervical problem. I will suggest that orthopedic surgeon be consulted to find a reason for new-onset cervical problem. It maybe related to low calcium and vitamin D levels in blood. These can be checked with blood test.

Meanwhile she can take the vertin tablet that may have been prescribed. Let me know which medicines got prescribed.

Also she must avoid any sudden movements at the neck. She should also avoid heavy work at shoulders or neck e.g lifting clothes or suitcases, kneeding atta dough, making chapatis, etc. Avoid head rotations, shoulder rotations. The movements that need to be avoided differ among patients. So she needs to figure out which movements she should not make. If wearing a cervical collar helps. then she can wear one. Avoid hot/cold weathers. Keeping the head straight while talking may also help. Avoid driving atleast until the episode is over.

PFA prescription.

For the pain on back side of head we used T.Nurokind G 100 mg twice daily for 2 weeks as prescribed. There is only unnoticeable pain now.

Only complaint is headache from few weeks intermittently and dizziness on yesterday and day before yesterday, nothing faced today.

Hello

In my opinion, Nurokind G can be discontinued.

She can continue with Tab Ibuprofen 200 mg as required or XXXXXXX 3 times a day.

In fact the dizziness that she is experiencing, maybe the side effect of Gabapentin (in nurokind G). So I think the dizziness may disappear within a week of discontinuing gabapentin.

Thanks for reverting back with details.

so she had severe neck pain, back of head pain 3 months back for which she was prescribed, Nurokind G, muscle relaxant, calcium and vitamin D. Nurokind G was discontinued 3 weeks ago. 2 weeks ago she developed lightheadedness. 2 days ago she developed head spinning and eyes rotation sensation.

I think she can also discontinue calcium and vitamin D as there is no evidence of its deficiency.

The precipitating cause seems to be muscle spasm in neck. It could have happened due to external trauma or internal cause like some lump pressing on muscle. Since you have not mentioned any external trauma, I think we should look for internal causes.
I will suggest an MRI or brain and neck that looks for nerves as well as blood vessels. A neurology consultation is also needed to look for any signs of focal neurologic deficit.
Meanwhile I recommend minimal medicines until a definite cause is found. Preventive measures as recommended in my first reply be taken.
