Having Polycystic Ovary Syndrome And Periods Are Regular. Taking Medication. Done With Tubes Test And Follicle Study. Advise?

Question: Dear Doctor,

We are trying to conceive for last 8-9 months.I am 29 yrs old and she is 26 yrs old. My wife has PCOD,and her menstrual periods are not regular. After taking ultra Sound doctor said that it is not critical. She is 163 cm and 50kg in weight. After the first scan she was advised to have Krimson 35, Diamet 500 and Utronorm (bottle) for three months. After that the doctor gave CCQ50, but unfortunately I was not in India-we were not together- during that time. After that she was advised to have B Long F , Diamet 500 and Fertisure F( Sometimes Oosure). During this period periods were not regular- Late by 7-12 days.Two moths later she was give clomipure50. Whenever she has clomipur she has a regular period that comes in 28 days. She had her first dose of Clomipure on 14th of March 2013.But she didnt get pregnant. A Tube test (Hydrotubation)was done on 18th of April and doctor said there is no problem with the tubes. The report says about 30 ml of fluid passed without any resistance.Altogether we had three cycles of clomid and she gets her periods regular on Clomid. Rest of the time its very irregular.
Semen analysis was also done in May and report is as follows
Vol: 1.6 ml, PH: 7.5, Rapid Progressive motility: 60%, Slow progressive motility 25%, Sperm Concentration: 99million/ml, morphology : 85%.Puss Cells: Less than 6
I am hypothyroid and the TSH is 17.13 uIU/ml in May and having Thyronorm 25.

A follicle study was suggested( Without Clomid) and on the 17th day of LMP follicle size was 16.5X11 and endometrium thickness was 9.5. on 20th day the scan said that the follicle ruptured. We had intercourse from 16th day till 20th day. But still she didn't get pregnant. So in that cycle she ovulated without clomid.
Even after that she didnt get her period on time, went to doctor and she suggested Susten 200. Had it for 7 days and she had her periods on 04th of August 2013 (LMP was 22/06/2013).
Please advise me on the following queries:
1. What is the chance that she can get pregnant?
2. In follicle study she had a ruptured follicle and had intercourse, but why didnt she get pregnant?
3. If she had ovulation on 11/07/2013 , why didnt she get her periods after 14 days. and why did she want to have Susten to get periods?
4. Are we on the right path of treatment?
5. Shoudl we consult ain Infertility specialist now?
6. Should we go for IUI?
7. What is the cost of IUI in India (on an Average)

1) There are high chances of your wife becoming pregnant as everything is apparently normal.

2) Susten was given by your doctor give support if there is pregnancy in that cycle. When you are taking progesterone suppliments like susten the cycle varies little bit and does not occur on the expected date. Its normal to have little variation in day of menses.

3) You are on right path of treatment.

4) It is always good to carry out procedures under guidance of infertility specialist.

5) IUI is next logical step and has highr pregnancy rates than trying naturally.

6) IUI costs approximately 5,000.

Thanks doctor for the detailed answer for my queries.
I have few more questions.
1. We are never been asked to do a blood test. Isn't it very important for infertility treatment?
2. Even after treating PCOD why doesnt she get normal periods without clomid?
3. Now we are under treatment under a gynecologist. When should we got and consult an infertility specialist?
4. She had clomid for four cycles and follicle study done once revealed that she ovulated without Clomid. But why doesnt she get pregnant?
5. Are my semen analysis show normal results? and also should I do some more tests?

1) Considering your age and egg count it is not always necessary to blood tests.

2) PCOD cannot go away completely .There is nothing called permanent cure. It will be there. Clomid will only help in growth of eggs and not regularising cycles.

3) Its always better to be treated bby an IVF specialist.

4) Ovulation is not only criteria for pregnancy. We dont know if ovulated eggg got fertilized. If fertilized, was it implanted. These are other steps before pregnancy.

5) Your sample is fine. You dont need any more tests.
