Hello!

Thank you for the query.

What do mean by "around the diaphragm"? Mostly by tumors around the diaphragm we do understand some lumps inside abdominal cavity like abscesses or enlarged lymph nodes.
I assume that you mean some lumps on your soft tissues of the abdomen. If so, such lumps can be lipomas or sebaceous cysts (which are very common at smokers). Only sebaceous cysts can be painful when get infected. Such lumps needs to be removed. The best way to get to know what are you dealing with is ultrasound. It is very hard to tell not seeing it. If you could attach the photo, it would be easier to help you.

Hope this will help. Feel free to ask further questions.
Regards.
