My wife's urine test report indicate 3-4 HPF pus cells and 15-20 HPF epithelial cells. She is complaing of low grade fever upto 99.5 in the evening. Is it urine infection or something else.

Welcome to the forum.
If there are other urinary symptoms like frequent urination, urgent need to pass urine and burning while urination, it is most probably UTI.
This needs confirmation with urine culture test.
Meanwhile, fever evaluation is a must and she needs to be examined clinically to look for a focus of infection, as fever is a symptom of infection somewhere in the body and it needs to be located for starting appropriate treatment.
Please consult a general physician asap.
