Question: Hi. I am married since past 6months
I have got fungal infection/ vaginal every month. I take candid CL 3 tablets everytime
This time me amd my husband both are feel burning sensation and i am getting urge to pee very often and it also pains. We also get itching feeling. How do we treat the burning sensation and what preacaution do we beed to take to avoid repeatatiom of this fungal/ bruise/ burning feeling

I can understand the amount of discomfort you have with the recurrent infection. First of all, allow me to throw some light on the origin of such a situation. Recurrent vulvovaginitis or infection of the vulva and vagina occurs when -
1. The initial treatment is incomplete. The local treatment should be taken for 14 days and oral medication should be taken for 7 days. When a shorter course is followed or treatment stopped as soon as symptoms subside, recurrences are common due to development of resistance.
2. Excessive use of antibiotics for other reasons can inhibit the natural resistance of the body leading to such infections.
3. Hyperglycemia. As is well known, diabetes mellitus can create immuno-compromised state leading to increased frequency of infections.
4. Use of local lubricants or spermicidal agents during sexual activity.
5. Sexual transmission through the male partner, rarely.
6. When there is lowered general immunity of the body.
7. Mechanical irritation to the part like tight fitting clothing or sexual intercourse, especially during the menstrual cycle.

If any of the above causes relate to you, you should get them sorted out. I would suggest culture test of the vaginal swab to see the drug sensitivity and go for the complete treatment. The treatment should be given to both of you and abstinence should be practiced until complete cure. Genital hygiene should be maintained during the menstrual period.

You should get urinalysis and get treated for urethritis as well. Urinary antispasmodic along with excessive intake of water can relieve the strangury (the urge to urinate often along with pain).

Your husband also would need an examination by a physician and if candidiasis is diagnosed, treatment of a complete course will cure the symptoms. Oral treatment will usually suffice.

Initial treatment should be followed by prophylactic treatment when there is recurrence. Prophylactic treatment usually implies monthly doses of oral anti-fungal agents for at least six months and several treatment protocols are available based on the consultant's discretion and the severity of the condition.

Meanwhile, improve your general immunity with good exercise, plenty of fruits, fresh vegetables and liquids. Use of probiotics can be tried.

I had got urine infection aswell a month back. Had got culture test done and had completed the course of medicine. Do i need to get culture test again? Should i show a gynac or general physician?

Flavoxate is used in the doses of 100-200 mg. 3-4 times a day. The dosage can be adjusted based on the intensity of the symptoms and the body weight. If the symptoms are severe and you are overweight, you can go for the higher dose. If not, you may opt for the lower dose. Frequency of the dose also depends on the severity of the symptoms. You can adjust the treatment as per your needs but do not exceed the higher dose. Please take the medication with food and plenty of water.

Alkaline citrate can be taken thrice a day at the dose of 10 ml. diluted in 100 ml. of water approximately.
