Question: Hello Doctor, I am XXXXXXX 31 years/Male/Unmarried man. I am planning for a marriage soon but some confusions related to future married life. such as: It is an arranged marriage, which is on on going process with couple of proposals a) One proposal where the girl is of age 29 years (Status: Quite positive) b) One proposal where the girl is of age 27 years c) Some other proposals are expected where the girls age are between 25/26 to 30. My questions are: 1) As I am 31, so which age of the girl as I have mentioned above will be better for married life related to Sex and fertility? 2) I heard from one of my friend as woman body losses sexual interest after age 27/28 years onwards and it affects their fertility as well. So will this be a risk if I chose a partner of age 29+? 3) High level planning for my future married life is: I wanted to make a family of 2 kids and as I am 31 then how to plan according for this objective if a) I chose a partner of age 29 b) I cose a partner of age 27 or laess than age 27 As all my qustions are little imaginative and fullfilment of the objectves are practical oriented but I still wanted suggestions from you which going to be useful to estimate my objectives Please suggest... Brief Answer: Age does not matter if she is less than 30. Detailed Answer: Hello, Thanks for trusting us with your health concern. The age of the woman does not matter as long as she is less than 30 years of age. The fertility depends on regularity of her cycles. Regarding the sexual drive it is present even after menopause in some women. The drive depends on the psychological conditions and hormonal balance of the body. It does not decrease till 35 years of age. You can choose any of the above mentioned options. It is best to consider their blood group- should not be Rh negative if you are Rh positive; family history of diabetes; stroke; cancers; genetic defects; malformed babies or any other morbid illnesses etc. If you want to have two kids it is best to complete your family before your wife is 35 years old as after that the chances of having malformed babies increases. Thanks Dr. XXXXXXX for a clear brief answer. Now after following your valuable response, I am with some more number of questions as mentioned down. Please help with yous suggestions..... As mentioned in my 1st query I am with 3 proposals of different ages like 25, 27, 29 and by keeping aside my mentioned concerns, I am pretty much positive towards the girl of age 29. Now the issue is, as you have mentioned that I need to complete my family by her age of 35 and by focusing that I fell that I am having very less numbers of years if chose her as my partner. Please guide me on the following queries. 1) My priorities are to start the married life in quite casual without any future responsibilities as I am not ready for the family now.I want to spend some good quality years with my wife and then I will plan for the family. Now by following this for 2 years, I will be 33 and she will be 31, when we will plan for our first kid and the outcome will expected when I will be 34 and she will be 32. After this how to plan for the second kid by considering our age? 2) If in case, things are not go well as I am planning now and we make our first kid by my age 33 and her age 31 then any risk factors will occur to have a healthy baby or malformed baby? 3) For the second kid, if we make slight delay like: my age will be 37-38 and her age will be 35-36, then any risk factors will occur to have a healthy baby or malformed baby? 4) In current medical practice, do we have medical approaches where late pregnancy with the risk of having malformed babies can be handled effectively with no risk? 5) As you have mentioned in your first reply as: "It is best to consider their blood group- should not be Rh negative if you are Rh positive". Please explain as not able understand clearly (My blood group is AB+) 6) By focusing to my married life priorities and current age factors as I am 31 and she is 29, please guide how to plan a good married life and family of 2 kids? Brief Answer: Detailed below. Detailed Answer: Hello. Thanks for writing again. 1. It is not necessary that she will conceive as soon as you will start trying. The process of conception may take 6 months- 1 year in some cases. It is better to start trying for conception when she is 30. The second kid should be planned at least 2 years after the birth of first kid so that there is a difference of 3 years between the two child births. 2. The risk factors do increase with an increase in age. These are more after the age of mother is more than 35 years. 3. You will have to get proper tests done to rule out congenital malformations after 35 years of age. 4. No. malformed babies need to be aborted if detected. 5. Your wife's blood group should not be A- B- O- or AB- 6. It is best start planning for a family after 1 year of marriage and plan the next kid when she is 34. Hope my answer is helpful. Hello Dr. XXXXXXX Thank you so much for the answers. By analyzing your suggestions, I felt quite confident now. As the age of both of us are quite on edge compare to my post marriage priorities but as you said chance are quite positive till her age 35. Just couple of more questions: 1) As you have mentioned that "It is not necessary that she will conceive as soon as you will start trying. The process of conception may take 6 months- 1 year in some cases.". Could you please advice on this as I am not able to understand this clearly. Is the woman need any specific time on which trying to conceive need to be followed for the pregnancy or to become a woman pregnant, she only has to be participate with her husband, while her husband is medically fit for to conceive her? 2) As you have mentioned that "Your wife's blood group should not be A- B- O- or AB-, as my blood group is AB+". Could you please advice on this as I am not able to understand this clearly. Any risk is behind of this if her blood group be A-, B-, O- and AB-? If any risk then, can it be handled medically or no options are available for the future issues? 3) In my day to day life in office, I see some of my female staffs who married quite late and also gone through the pregnancy and delivery quite late like after age 35. But the good part is, they all quite healthy and there is no issues also with their kids. So my question: Are there enough medical options available if some body wanted to have a healthy child after age 35? Please guide.......... Thanks, XXXXXXX

Brief Answer: Detailed below. Detailed Answer: Hello. 1. Even if both the partners are healthy and fit the chances of conception per cycle are around 20%. It is best to have intercourse daily during the fertile days- Day 11 to day 19 in a regular 28 day cycle to improve the chances of conception. 2. If her blood group is negative type then the second baby may have problems like jaundice. But this can be handled well if proper treatment is given during first delivery. 3. Yes. We have adequate supplements and screening tests which help in having a healthy baby even after the age of 35. So do not worry and go ahead. Regards.
