Question: When I lie down and get up I feel like fluid I'd swishing in my head . It is sometimes accompanied by a loss of balance particularly getting up. I think it may be vertigo but it is persistent and disappears for awhile and comes back again. Any ideas ?

Brief Answer:
Most likely BPPV.

Detailed Answer:
Hi,

Thank you for your query.

1. This appears to be BPPV (Benign Paroxysmal Positional Vertigo).

2. This is the most common cause of peripheral (ear related) vertigo. This is caused by displaced calcium particles in balance system (labyrinth) of the inner ear (semicircular canals).

3. You need an ENT and Neurology consultation. Certain signs such as nystagmus (abnormal eye movements in vertigo) can only be observed directly.

4. Your doctors need to check the ears, hearing (Audiometry), a Neurology examination, Gait testing, Caloric test, Dix Hallpike test, sometimes an VNG (Video NystagmoGraphy) amongst other tests to arrive at a diagnosis.

5. BPPV responds well to treatment. The Brandt Daroff exercises, the Semont's maneuver are better initially followed by the Epley's maneuver. Use a DizzyFix device to repeat the Epley's Maneuver.

6. Let me know the progress.

I hope that I have answered your queries. If you have any further questions, I will be available to answer them.

Regards.
