Posted on Mon, 25 Jul 2016
Question: Dear Sir,
I am an XXXXXXX Male, presently staying and working in Abu Dhabi. I have given all the details as much as possible for your kind advice and further directions.
Age: 34 Years (16-9-1981)
Height: 166 cms (5 ft 6 in)
Weight: 94.8 Kgs (BMI 34)
Work & Residence Location: Abu Dhabi (UAE)
Work Condition: Almost Sedentary
Past Medical Conditions Description:
(1)     Suffered from RENAL CALCULUS in 2003 due to OTC medicine used for Hydrocele
(2)     Suffered from HEPATITIS (don’t know which one) in 2004 which was an epidemic happened in my hostel due to contaminated water and almost 5 people suffered at the same time.
(3)     Diagnosed with FATTY LIVER – Grade 1 in 2013 due to heavy usage of Alcohol.
(4)     Since last 6 months, numbness and tingling feeling on left neck nerve which is aggravated in the night time with numbness on left face.
Lifestyle:
(1)     Used alcohol heavily at the tune of 150-200 ml daily from 2008 – 2012 while staying as bachelor with friends.
(2)     After Diagnosing Fatty Liver in 2013, I reduced alcohol consumption to weekly affair where in consumption was almost binge for two days as weekends.
(3)     Now, since last 1 year, due to fear about liver, and occasional discomfort in liver area, Alcohol consumption is only a weekly affair of XXXXXXX 200 ml total till 18th XXXXXXX 2016. After which I have not touched alcohol.
Present Medical Condition:
(1)     On 22nd XXXXXXX morning, I did some XXXXXXX Namaskar which was not a routine affair but due to which I started getting pain in right lower rib cage area which was distended towards right kidney area at the back.
(2)     On 23rd XXXXXXX evening, I got to meet a Gastroenterologist, who after comparing with my previous reports of March, and seeing my BP on that time at 145 / 95 and pulse rate at 97, he wrote for new reports and results of both the reports are attached in Date/Column wise.
(3)     On 29th XXXXXXX I did my ultrasound and Gastroenterologist prescribed following medicines
Present Medications:
(1)     Concor Cor 2.5 mg (Once every Morning X 90 Days)
(2)     Euthyrox 25 microgram (Once every Morning X 90 Days)
(3)     Ursofalk (Ursodeoxycholic Acid) (Twice a Day X 60 Days)
(4)     Movicol (One XXXXXXX at Bed Time X 20 Days)
(5)     Cyanocobalamine (1000 mg) via injection every 2 weeks)
(6)     Cyanocobalamine has been changed by me to Methylcobalamine (500 mg) tablets which I started taking 3 times a day (total 1500 mg) and now my neck nerve problem is far better relieved which I was not able to get after using one dosage of Cyanocobalamine injection. Its worth mentioning that this shift from Cyanocobalamine Injection to Methylcobalamine is not under the supervision of Doctor.
My present symptoms after taking above medicines:
(1)     BP is 180/27, pulse rate is 67 (Concor Started on 24th XXXXXXX and BP rechecked on 29th June).
(2)     Aggravated symptoms of frequent sneezing and bouts of Asthama which prompts me to use Asthalin by Cipla
(3)     By the usage of Ursofalk, my discomfort in liver area is not there but I use to get occasional diarhoea.
(4)     Neck Nerve pain and discomfort relieved upto a maximum extent.
(5)     Kidney area discomfort increased
(6)     Normal Appetite
(7)     Reduced One Kg in One week under food control and no alcohol usage.
(8)     Occasional Dizziness
My Questions:
(1)     Is it very necessary to use Concor Cor at this age of mine (34 years)?
(2)     Does Ursofalk cause any sort of sideeffects to liver on a long run?
(3)     Do I have to completely stop Alcohol or occasional drinking is fine?
(4)     Does minor Asthama Attacks is due to Concor Cor and if yes then use of Asthalin Inhalers can cause contraindication with Concor?
(5)     Upon your expert review and evaluation of above said details and correlation with reports, what is your inference on long term chronic diseases of liver, ie, can you advice whether I am suffering from Fibrosis, Cirrhosis or any other liver disease.
(6)     As I am doing diet control, so can I take Multivitamins (Ranbaxy Revital or Vitabiotics Cardioace) once daily or will it contradict with above said medicines?
(7)     Last but not the least, If you feel that the above medicines are not necessary at the age of 34, then kindly advice your expert opinion on lifestyle changes or medications which may not harm on a long run.

Hi,
Thanks for posting your query.
I am Dr.R.K and I am pleased to assist you.

Are you from healthcare industry?

1. You have mentioned that your doctor prescribed Concor cor 2.5. He is right in prescribing that as you had high blood pressure and the pulse rate was on the higher side.

2. No Ursofalk does not cause any side effects on liver on a long run

3. You have to completely stop drinking alcohol

4. Asthma attack is less likely to be due to Concor Cor as it is a selective beta blocker medication. The incidence of asthamatic attacks with selective beta blocker is very low.

5. You are not having cirrhosis. To know whether you have developed fibrosis you need to undergo fibroscan.

6. You can take multivitamins

7. Your doctor has prescribed the right medications. Take them as advised. Stop drinking alcohol.

Regards,
Dr.R.K.

Thankyou very much Doctor for your reply.

I am a B.Tech, MBA graduate and not from healthcare industry but i use to read a lot about health and fitness and various streams of healthcare like alopathy, homeopathy and ayurveda. Also Started getting interested towards yoga.

Sir, as i can see from your reply, that i am not suffering from Cirrhosis, and can fibrosis can be detected thru blood test (already attached)?

Secondly, my elevated enzymes in blood test can cause any problem (already attached)?

Thirdly, i beg your pardon to ask you this question which i am not supposed to do, as i will be going to XXXXXXX on 6th till 21st July for my sister in laws marriage which will be having drink parties. Although i am very much concerned about my health and has not touched liquor since last 20 days and fully bound to continue the same for future also. But, my concern is can i take Two beer or not more than 100 ml hard liquor while in marriage function bearing in mind that i will not take either concor or ursofalk on that day?

Fourthly, i have heard that stopping concor abruptly can cause adverse effect. what does this mean, ie, if i take it for 1 week and stopped for 2 days, will it cause any adverse effect?

Gentle Reminder....

Hi,
Thanks for getting back to me with more questions.

1. Fibrosis cannot be detected with the blood test reports which you have uploaded here.

2. Only ALT is mildly elevated and is not significant and is not a thing to be worried about.

3. I would advise you against taking alcohol

4. Only those who are on long term Concor Cor should not stop it abruptly.
You are on Concor cor for about a week only. So there is no need to wean it off.

Regards,
Dr.R.K.
