Question: Hi My son have Chest cunjestion and Infection (after blood test), doctor has given Claritt (
Clarithromycin) on 21st March and given antibiotic Claritt (Clarithromycin) Amydramine, Mucum and one for fever.
I want to inform till today in the morning his fever is 101.50 to 102.5, means fever is constant ( down for 1-2 hour after medicine).
He is not taking any food because he vomits most of the time.
Please suggest shall I change Antibiotic to Clavam Forte or any other, as my son fever is not coming down which means bacterial infection is also not resolving. It's already 5 days past.

Brief Answer:
Rather than changing antibiotics he needs evaluation

Detailed Answer:
Hi.... you have mentioned in medical history that he is always drowsy and not taking any food. More over he is a vomiting the medicines which you are giving also.

It is very clear that the medicines are not working, you are right. But at the same time changing the antibiotic without proper investigation or finding out what is happening with your kid is not the correct way of approach.

If I were your kid's pediatrician I would suggest doing basic blood workup with CBC, CRP, blood culture and also a chest X-ray will be definitely required.

I suggest you get back to your pediatrician with this tip and request for further workup and management.

Your kid might also be required to be hospitalized and started on intravenous antibiotics.

I real but your kid might be having very severe pneumonia, as the kid is not responding to medicines even on fifth day and is running high grade fever still.

Brief Answer:
I have gone through the reports - I suggest hospitalization

Detailed Answer:
Hi.... I have just now gone through all the reports you have uploaded.

The x ray report or X-Ray image is missing in the reports. But by what the current reports suggest I feel that your kid might be having a lower respiratory infection or an atypical infection which will require further management.

Your doctor is perfectly right in treating with Clarithromycin, but unfortunately the kid has not responded well to that particular antibiotic.

So I suggest that he be hospitalized and given intravenous antibiotic like ceftriaxone along with oral antibiotic like Azithromycin in the appropriate dosage and duration and schedule.

I am suggesting hospitalization and intravenous antibiotics because today is day 5 or fever and it is high grade.

Brief Answer:
Looks like either viral illness or atypical infection

Detailed Answer:
Hi....I have gone through the latest reports and x-ray also.

If you carefully note...the differential count has changed with predominantly polymorphs to raising lymphocytes now. This is suggestive of either a viral or atypical infection.

Moreover the total count also has reduced. This suggests that the kid's infection is settling.

I have a few questions for you -

1. How is the fever now?
2. How is his activity when the fever is touching the baseline?
3. Is there any respiratory distress or fast breathing?
4. How is the feeding now?
5. Has he been hospitalized?
6. Has any blood culture been done?
