Does An Abortion Affect Fertility And Cause Difficulty To Conceive Again?

Question: Hi, My wife has a bicornuate uterus and she is two weeks pregnant. We are not ready for the child but need to know the repercussions for conceiving again if we dont plan the child now? Does an abortion affect her fertility to conceive again? Shall we start for curing a biiconuate uterus?

The main problem in bicornuate uterus is preterm delivery or mid trimester abortion.
But to predict that is very difficult and needs MRI evaluation to determine the need of correction.
Abortion in your case will not affect future fertility but in case the abortion is incomplete then it might need evacuation surgically which might be difficult because of bicornuate uterus.
In my opinion you should continue this pregnancy but if not possible to continue the same then visit a specialist so that you are in safe hands.

Can you tell me a specific reason of your suggestion to "continue pregnancy". Is this recommendation somehow relates to the health of my wife. Also, our doctor stated that abortion, if done, will be medical and not surgical as its only two or three weeks of pregnancy....is my doctor correct in saying this? We shall be planning a child after two - three years...will this mean that there shall not be any problem of conceiving again taking into consideration that we abort now. Also, if we abort now than do we need to advise doctor for resolving the bicornuate uterus issue.

Hello sir, the only reason to continue this pregnancy is the mild risk of surgical curettage in case medical abortion is incomplete.
Risk of incomplete abortion is always there in all pregnancies terminated by medical abortifacients but risk of perforation is more in bicornuate uterus.
This risk decreases if curettage is done under ultrasound guidance.
Her future fertility won't be affected.
Your doctor is correct in saying medical abortion is the right method of termination in your case.
No the bicornuate uterus correction if needed is never done on pregnant uterus.
Once your wife is free from pregnancy then she will be evaluated by MRI and then decided for evaluation.

One last question:

What is perforation risk? And what precautions are required to address this risk. Also, can you please tell me what i should tell my doctor in case we decide for abortion (curettage under ultra sound guidance ?)

The perforation risk is 5-7 % in normal uterus which increases by 1.5-2 times in case of bicornuate uterus.
To prevent this the only precaution is to take it under ultrasound guidance.
