Question: I would like to know if it is possible for a complex ovarian cyst that is septated with thick walls that has been present for 3 years, to become a simple cyst.

Hello,
Thanks for the query to HCM,
If the cyst hs been a complex ovarian cyst and then with thick septations, then it cannot become a simple , non separated follicular cyst.
At 49 years of age, a simple cyst is unlikely.Also confirmation a complex cyst should be made with an MRI .A complex cyst can be an ovarian cancer and would require tumour markers to be investigated like CA125, CEA, AFP, BETA HCG., which is mandatory.

I do not have the US from last week, as it was a hand written report from the US tech, just said, cyst was 4 cm x 3 cm and was a simple cyst. I didn't believe it.

the ultrasound attached with the question clearly states the presence of a complex cyst with septations of size 4x4 cm in size in the left ovary where the cyst occupies a large area and the remnant ovarian tissue is left peripherally .
This is most likely to be a ovarian tumour.
Please get the tumour markers done,

Thank you very much, would a MRI be a reasonable follow-up? The CA-125 has been done, but I also know that this test can be very inaccurate.

Yes MRI FOLLOW UP IS MUST . Also all the 4 tumour markers have to be done before establishing a diagnosis. CA-125 more than 200 along with raised CEA would indicate a ovarian cancer, though staging is always surgical.
