Does Truvada Prevent HIV Infection?

Question: Hello Dr.,

I am a 42 years old man and unmarried.
For my sexual needs, I am planning to start relationships with the unknown females from different sources.
Need your guidance, so that I can protect myself from HIV during the plays.

1) Is it fine, if I will start TRUVADA for a month and then start the sexual relations with the unknown females?, so the risk of HIV will can be 0%, even if I am involve in the play by without any condom and the unknown partner is having HIV.

2) Is TRUVADA 100% assured precaution to HIV infection?

3) If TRUVADA is not 100% assured precaution to HIV then will this be a good approach to use condom in every play? Is there any risk, if I use condom during
the play and the unknown partner is HIV infected?

4) Why it is advisable to put off the condom after play immediately? It could be valuable for avoiding pregnancy but for to avoid HIV infection, putting of the condom immediately after the play has the bigger chance of the condom slides fully over the erect penis surface, where there is a chance of contact between the opening of the penis with the base of the condom, which could be with some vaginal fluid during the play.
In this case, if the vaginal fluid at the condom base get contacted with the penis opening, then is there any risk of HIV to the male partner, on the condition as the female has HIV?
Is it a good approach to put of the condom after some minutes of the play, so that if the vaginal fluid is at the base of the condom then the HIV cells will be dead after some minutes and then putting off the condom from the penis could be more safer, even the vaginal fluid contains HIV cells?

I understand your scenario but do not approve of your plans. I do not find it a good idea. Rather, it would perhaps be a better idea to enter into relationships that need not necessarily culminate in marriage. You mistake your needs only to be sexual. A stable relationship has much more to offer than mere sexual pleasure. If you are planning to experiment, experiment in relationships and not only media of sex.

Let me try to address your queries one by one:
1. and 2. It is not fine. in their site https://start.truvada.com they clearly state: Just taking TRUVADA for PrEP every day may not keep you from getting HIV-1 infection. An HIV prevention plan includes safer sex practices and the correct use of condoms during any kind of sex.
Now I wonder, if you are already protected (using barrier methods or other preventive modes) why are you really going to go for Truvada at all?

3. As you already know that Truvada would not be a good idea and so you MUST use condoms on ALL occasions. It would not matter if your partner is HIV positive as the barrier method relates to maintaining barrier from all, irrespective of the HIV status of the person.

4. Get the basic idea. The basic idea is to not unnecessarily put the contaminated condom on. If you put it on after the play, you risk touching the partner side of the condom to your own body and thus the isolation from the partner secretions would no longer be complete. In such a case the probability of transmission would no longer be reduced to zero. The rest is upon you.

Please note that assuming your partner to be HIV positive and assuming that the skin over your body does not have raw surfaces, rubbing your body, hand or other parts against each other will not transmit HIV. However, it is not the same for bodily secretions contaminating each other's.
