Question: My son is taking Subutex. They upped his dose last week and since then he's been having major panic attacks. Is this a side effect? Do they need to lower his dose ?

Brief Answer:
Could be due to Subutex dose

Detailed Answer:
Hello
Thanks for asking

Subutex consists of Buprenorphin. The drug is used in variety of conditions especially in opioid dependence.

He has developed panic attacks on increasing the dose of drug. Buprenorphin has potential to cause side effects like pin point pupils, giddiness, nausea, itching, dry mouth, postural hypotension.

Sudden increase in dose might result in postural hypotension which may result in symptoms of giddiness and anxiety. Panic attacks could be due to increased dose.

I would advise you to check his pupils in a well lighted room. If they are pin point small and poorly reacting to light then most likely his doses are in higher range. Secondly get his BP checked in lying down position and then immediately in standing position. See the difference in BP. If there is significant decrease in standing position then his problem is due to postural hypotension. This is time to decrease dose and consult his doctor again

Thanks, hope this helps you. If you have more doubts you can ask again.

What if and I don't know that he has, im just asking. What if he took some opioids while on the Subutex. Could that cause the same panicked feeling?

Brief Answer:
Will result in additive effect

Detailed Answer:
Hello again

Subutex is itself an opioid drug. If he has taken some other opioid along with Sututex this could be one of reason of his severe panic attacks because actions of two same drugs become additive resulting in increase in severity of side effect.

Thanks, hope I helped you. If you have still more unresolved doubts you can ask again.
