Does Lurasidone Interact With Zapiz And Quetiapine?

Question: Went to my doctor yesterday in XXXXXXX he made the following changes:
Lurasidone kept at 40
Amitryn kept at 25
Sizopin kept at 12.5
Zapiz at 0.5, all in the night
He changed quetiapine from SR 100 to IR for better sleep
I have stopped zapiz 0.125 and inderal la 20 in the morning as my anxiety was lil better.
I have following queries:
I am getting breathless while speaking today and can't pronounce words which are tough as my tongue twists while pronouncing them, is it because of change of quetiapine from SR to IR? If so, will it settle down with time?
Didn't sleep last night after changing quetiapine to IR? Will it improve with ir? If yes, how long will it take?
What else can I do for my sleep? And what are the side effects of changing quetiapine 100 SR to IR
How long does lurasidone take to show full effect? I have been on 40 (increased from 20 to 30 to 40) for the last 4-5 days, will my breathlessness stop with time on lurasidone?

Why they have started Sizopin again?

The slight breathlessness or twisting of tongue while talking are probably occurring due to anxiety. These are mild symptoms and ignoring these symptoms will make you better. Just try not to give attention to these symptoms. Avoid catching breath again and again. Don't take rapid shallow breathing. Shifting from SR to IR Quetiapine is not the cause of these symptoms. Just try to ignore these and this will settle with time.

Yes sleep will improve with time. SR tablets result in maintaining the sleep. IR acts rapidly but the sleep will improve with time.

Changing from SR to IR doesn't cause any serious effects except sedation.

Lurasidone will work in controlling the symptoms, just wait for few more days. The breathlessness will stop with time while on Lurasidone.

Hello doctor- didnt sleep well last night also. Sizopin 12.5 was always there. I have following queries:
if quetiapine ir 100 is given to me for sleep, why is it not helping me and how long will it take to help?
Zapiz 0.5 in the night does not seem to be helping me with sleep! what do i do with it, should i stop it?

Hello again,

Quetiapine helps in sleep but some times higher doses are needed to get desired effects. If you are not taking Zapiz then increasing the dose of Quetiapine can help in sleep. This will improve the sleep.

If you are getting hiccups and uneasiness immediately after taking IR then that could be the reason. But that would cause only mild symptoms.

Lurasidone doesn't cause sleeplessness. It has not very strong effects on sleep. Wait for a week and see if improvement occurs. If symptoms persist then increasing the dose of Lurasidone is option.

For sleep if possible talk to your doctor and ask for increased doses of Quetiapine. Alternatively increasing the dose of Amitriptyline is another option. This will help to regularise the sleep.

IR helps in immediate onset of response while SR helps to maintain long duration of effect. IR or SR can both be taken, SR will help in refreshing sleep. Half life is about 7 hours while for SR its about 10-12 hours. This difference is not the reason for breathlessness you're having.

Steady state concentration is the level that a drug achieves in brain to provide optimal effects.

Please wait for see the full effects, the drugs will control the symptoms.

Please try to ignore the symptoms if possible. Due to anxiety you are experiencing the symptoms in high severity this is resulting in more anxiety and a vicious cycle is resulting in side effects.

Yes increasing the dose of Amitriptyline is a good option. The drug will also regulate the sleep rhythm and the sleep full of depressive symptoms will decrease. For some days you can take antacids with Quetiapin. Pantoprazole or Rabeprazole will treat the stomach upset symptoms and the drug will become more tolerable.

It can't be quantified that 50 ir is equal to 100 sr but as I have mentioned in last answer that in terms of acute onset of actions 50 IR is almost equal to 100 SR. So even on abrupt shift you will not face any withdrawal symptoms. It can be said that 50 IR is equal to 100 SR in acute actions.

Please continue taking Pantoprazole D empty stomach in morning and in evening you should take 20 mg Pantoprazole plain about 2 hours before dinner. Pantoprazole is longer acting and in normal cases only once a day doses are given, but as you are having stomach issues so evening dose would definitely do good. In your case morning and evening doses would do better.

Lurasidone is a newer antipsychotic drug. The drug is given in your case for resistant anxiety and depressive symptoms.

Amitriptyline is for sleep and anxiety.

Sizopin is probably also given for sleep because the drug has great sedation. But its also used in resistant symptoms.

Zapiz is for anxiety and sleep both.

Quetiapine SR is for sustained control but right now your main issue is sleep and IR will be better.

Zapiz results in reduction of anxiety and causes hynosis which results in sleep. But increased REM duration leads to weird dreams. In most individuals onset of sleep compensates for it.

But as the dream problems are distressing in your case. In such case I'd advise you to reduce the dose to 0.25. No need to go to 0.375, you will not face withdrawal.

Yes you can increase to 35 mg and then if needed go to 50 mg. This will help in sleep.

Yes constipation., blurred vision, dry mouth, urinary retention etc might be seen with it. But these are seen in very high doses. The drug will not increase weird dreams. I am prescribing this drug to a lot of patients in my clinic for insomnia.

Quetiapine, Sizopin, Amitriptyline and even Lurasid have anticholinergic effects which result in dry mouth, blurred vision, constipation etc side effects. The tremors could be occurring due to three antipsychotics you are taking.

Reducing SR 100 to 75 IR is not the reason for the symptoms you are having.

Reducing the dose of Quetiapine to 50 mg will not cause any withdrawal symptoms. But this should help in dry mouth, blurred vision, constipation etc symptoms.

Sizopine also results in good sleep. But the drug also causes dry mouth, blurred vision, constipation etc side effects. But I'd advise you to talk to your doctor and ask for complete dose reduction of Quetiapine and if possible Lurasidone. Increasing the dose of Sizopin and stopping Quetiapine and Lurasidone will help in better functioning. In my opinion giving three antipsychotics simultaneously in subtherapeuctic doses is not a wise thing compared to giving Clozapine in good doses. This will reduce the side effect risk and will help in better functioning.

Lurasidone is not the resin for disturbed sleep. You are on very low dose and I don't think Lurasidone is the reason. Sleeplessness in your case is probably due to persisting anxiety and may be due to Zapiz.

I prefer to advise it in evening but later 40 MG twice a day is given. You can take it in morning without worries.

Good eve doctor - My medication is given below:
Quetiapine 50, sizopin 25, amitryn 25, zapiz 0.25, lurasid 40 all in the night. During my last visit to the doctor on tuesday (24th Jan), quetiapine was reduced to 50 from 75, sizopin was increased to 25 from 12.5 and zapiz was reduced to 0.25 from 0.5. With this change, i have observing internal shakiness (though it was there earlier also, it started with changing quetiapine from sr 100 to ir 100 and continuing after making the change in the medication), will it go with the time.
Also, getting excess salivation and while speaking today in the office, saliva came out and became conscious after that and went to see the doctor and he has added glycolate 2 mg morning/eve
I have following queries:
Can i take something for my shakiness, have tried pacitane and kemradin earlier but they would give me bad blurred vision, is there something else i can try which is safer?
Does glycolate help with shaking also? do i need to have 2mg mor/eve or i can start with 1 mg mor/eve or just take 1 mg in the morning to begin with? does it have any bad side effects and does it interfere with antacid as i take pantocid mor/eve?
Read on the net that amitryn also help with excess salivation, is 10mg amitryn an option? will it increase my shakiness as it is known to cause tremors?

Increasing the dose of Sizopin is a good move. In future request your doctor to stop the Quetipin completely and if needed increase the dose of Sizopin. You are already on three different antipsychotic drugs.

Glycolate will help to beat the shakiness, salivation etc symptoms. The drug is safe to use. Even it can also cause mild dryness of mouth but that would be tolerable.

Yes this will help in shakiness too. Start with 2 mg morning and afternoon. This will be best option. The drug will not cause any bad side effects. If you are getting anxious then you can start with 2 mg in morning for about 5 days then start taking afternoon dose too.

Amitriptyline due to anticholinergic effect helps in salivation. But that effect is seen in high doses and not at 10-25 mg dose.

Half life of oral Glycolate is about 3.5-4.5 hours. This is the reason I am asking you to take morning and afternoon dose. In evening sleep will compensate.

They shakiness and trembling in your case is occurring with two possible reasons, one is persisting anxiety and other is due to extrapyramidal side effects due to the antipsychotics you are taking. Though the side effects are not commonly seen with second generation antipsychotics but with three drugs the risk increases.

Glycolate will reduce the cholinergic side effects and will reduce the trembling and shakiness. No need to worry.

Its better to reduce the dose of Zapiz. Zapiz or Clonazepam is known to increase REM sleep and thus increases weird dreams or nightmares. Amitriptyline and Lurasidone though increase the risk but that is compensated by their antidepressant effects. Overall antidepressant action improves REM latency and thus help in refreshing sleep.

Brief Answer: Lurasidone will work with time. Detailed Answer: Hello again Thanks for asking again. Why they have started Sizopin again? The slight breathlessness or twisting of tongue while talking are probably occurring due to anxiety. These are mild symptoms and ignoring these symptoms will make you better. Just try not to give attention to these symptoms. Avoid catching breath again and again. Don't take rapid shallow breathing. Shifting from SR to IR Quetiapine is not the cause of these symptoms. Just try to ignore these and this will settle with time. Yes sleep will improve with time. SR tablets result in maintaining the sleep. IR acts rapidly but the sleep will improve with time. Changing from SR to IR doesn't cause any serious effects except sedation. Lurasidone will work in controlling the symptoms, just wait for few more days. The breathlessness will stop with time while on Lurasidone. Thanks
