I have gone through your query with diligence and would like you to know that I am here to help. Imodium (with the active ingredients loperamide) is an anti-diarrheal medication and not in any way a metabolite of any opiates. Drug screening tests that test for opiate look for morphine in the test result, and in no way would imodium (loperamide) test positive for this.

If at all you were on only this medication and your test returned a positive result, then this was a false-positive and I would recommend a mass spectrometry based method which would be most accurate. But even still, declare the imodium on the UDS, this will make things more simpler.

The tests look for opiates with mostly morphine as the target drug, and anything similar around it. Rest assured, if the test is conducted by an experienced laboratory and skilled personnel, it shouldn't result in a false-positive. As explained earlier sir, they should provide you with a declaration form before the test, wherein you can mention what medications and/or drugs you have taken lately, therein, mention imodium (loperamide) and the sample provided by you should be tested to exclude any similarities or discrepancies caused by the drug imodium.
