Question: To be answered by Dr.S XXXXXXX
Sir as you already know my case my high risk exposure was in Nov 2013 and my 1 st test( tridot 3 Rd generation)was in may 2015,2 nd test July 2016( 4 rth generation ag/ ab test) and 3 Rd test was in March 2017 by the same 4 rth generation test and all were non- reactive. And all along I was on several medications like ORGAMED/MEPRATE/PROVERA/DRONIS 30/METFORMIN/SYNTHROID/CLOMIPHENE CITRATE/ANTI- HISTAMINES /PRENATALS(folic acid and vit d supplements ( for conditions like PCOS,HYPOTHROIDISM,INFERTILITY AND POLLEN ALLERGY .I was on all these drugs ON AND OFF from XXXXXXX 2007 till MARCH 2017 (and my exposure was in Nov 2013)some at the time of testing and some inbetween tests. So I am doubtful about my test results because if my pattern of drug consumption and the drugs I am on is producing false negative results. Is there any chance of my test results turning positive after 3.3 year of exposure due to my pattern of drug consumption? Please clarify on this part sir I am unable to concentrate on my life. What is the maximum window period of hiv if I am on progesterone drugs and the drugs I mentioned ? how should I calculate the 6 month window period.. is it from the day of exposure or from the last day of taking progesterone drugs after the high risk exposure?? Do answer all my questions sir. Will be much appreciated.

Brief Answer:
Beyond 6 months after the exposure seroconversions is unlikely.

Detailed Answer:
Hello xxxxxx,
Welcome once again to HCM.
I already clarified that the maximum extent of HIV window period is just 6 months only even in presence of immunosuppressive drugs, not beyond this. There is no possibility of seroconversion following 3.3 years after the risk activity, unless there is a new fresh infectious contact. 6 months is from the date of risky sexual encounter. Definitely not after the stoppage of suspected drugs.
Don't get nervous. In your case I already assured that you are free from HIV infection. Relax and concentrate on your normal routine and enjoy your days ahead.

Sir can paracetamol,painkillers/analgesics taken for menstrual cramps, headaches, cold and cough affect the test results ?
doctor
Answered by Dr. Sankaranantham Murugan (3 minutes later)
Brief Answer:
Paracetamol is very common drug, has no influence on antibody formation.

Detailed Answer:
Hi,
Welcome to HCM.
Definitely not. Moreover you had your tests after 1.5 years after the exposure and where came the question of delay in antibody formation or a false negative result? Anything beyond six months could not be possible.

Sir this is what a ayurvedam specialist says about zinda tilismath.
"zinda tilismath can be used both internal as well as externally as reference by the manufacturer.
Possibility of influencing hiv test by using externally is Less significant.
As internal usage may influence since the product contains good immune modulating drugs like black pepper, cinnamon etc.
but scientifically no evidence.
Zinda Tilismath has got rich medicinal properties. The basic compound of course is Eucalyptus oil and the other ingredients include cinnamon, cloves, mint, loneple, black pepper, cardamom, alum, ferric oxide and omum seeds.
Even though there is no proven studies available for the product against hiv antigen. But they still potent enough against the Tcell activities.
Hopefully tilismath may have influence hiv test for higher risk subject.
It didn't have any detailed scientific research update till date. traditionally they are potentially enough to fight against hiv infection.
continuous oral intake may influence the subject. If you are still not on continuous medication may show false negative after 6 months of high risk exposure. But it depends on your immune response.we cannot conclude it is because of the drug. (zinda).zindatilismath influence the hiv test and it's results.Taking zinda tilismath for a acute course may or may not influence the test results. During the course of time your body own immune system may produce antibodies against the infection if it exist. So it shows non-reactive. Even though you exposure for high risk it does not matter. If you are taking the lab investigation during the course of medicine intake. The sero conversion delay may occur. As active principles of black pepper (Nevirapine) is a potent non-nucleoside inhibitor of HIV-1 reverse transcriptase and is indicated for use in combination with other antiretroviral agents for the treatment of HIV-1 infection.zinda It is immunebooster drug. Not a suppressor.overall,Individual drug in zinda got potent effect on hiv infection. Certain Clinical studies with single or combination drugs showed good results. But microbiological aspects on sero conversion and diagnostic accuracy studies were not established against these drugs."And asking me to retest again after 6 months just to be on the safer side. DO I NEED TO RETEST AFTER 6 MONTHS? I will take your word for granted sir please let me know your take on this.he gave a really confused answer and I am panicking again. Please help me sir. Are my test results conclusive?? Zinda tilismath is a unani medicine popoular in XXXXXXX

Brief Answer:
You need no test for HIV.

Detailed Answer:
Hi,
Welcome back.
Immune boosting drugs will fasten the antibodies formation. Only immune suppressive drugs can delay the seroconversion.
I am handling HIV patients since 28 years and updating myself in the field of HIV medicine regularly by attending national and international HIV conferences as faculty and also through net.
I feel that you need no further testing for HIV.

And what is that black pepper ( nevirapine) he was talking about ??? Clarify this as well sir thank you
doctor
Answered by Dr. Sankaranantham Murugan (5 hours later)
Brief Answer:
Nevirapine has no relation Black pepper. No common property.

Detailed Answer:
Hi,
Welcome back.
I am telling that you need no further tests for HIV, is spite of you had taken that Aurvedhic drug..

Nevirapine is an ART drug belongs to the category of Non-nucleoside Reverse transcriptase Inhibitor. It is a purely synthetic drug and has no relation to XXXXXXX Nigrum (Black Pepper). Black pepper contains an ingredient called as Piperine. It is not anyway related to Nevirapine. Black pepper has a property to enhance the bioavailability of Nevirapine in the body to an extent. That is all. Black pepper has no anti-retroviral effects as such. Even if you are on such a drug, seroconversion to HIV beyond six months is quite unlikely.
