Question: Small Fibre Neuropathy

Hello Doctors,
I m asking about my neurological problems again. I recently came to know about SFN and little search about it revealed that it happened to those who have autoimmune disease like sjogren and systemic lupus.
My blood test reveals that I have high SSA antibodies but I don't have sjogren based on doctors clinical examination schrimer test and salivary glands biopsy. Not sure if I ha e lupus or not but doctors think that SSA antibodies in my blood is false positive.

My problems are chronic paresthesia that can last to hours up to few days goes away and come back. Sometimes a slight burning in right hand, numbness and tingling when I hold heavy shopping bags in my right hand and it also triggers paresthesia all over my body . When I sit cross leg my foot gets prickling and triggers paresthesia all over my body. This happened with both legs. Also when there is pressure on hands of causes tingling and again paresthesia all over my body.

A little history about me.
I had carpel tunnel 3 years ago but it was fixed by splinters. Recent NCS and brain MRI are ok. Lumber spine has scoliosis concave to right. Cervical spine has scoliosis to left, c4-5 mild disc bulge with foraminal protrusion to the left of anterior subarchanoid space, and hyperintense signal loss for all the cervical disc.l suggestive of disc dessications. My neuro surgeon believes that my symptoms are not related to neck or back problems but still he is unsure and send me to physiotherapist for fixing that mild disc bulge in the neck.

So could this be small fibre neuropath problem ?

Sjogren disease may have small fiber neuropathy which presents with sensory symptoms like pain and paresthesia.
You can repeat the blood test for Sjogrens if in doubt.

Dear Doctor.

Two times I got tested for ssa antibodies and both the time it came strong positive.

I don't have dry eyes and mouth as evident by schrimer eyes test and salivary glands biopsy

You should get an evaluation by a neurologist.
Take drugs like pregabalin or gabapentin with methylcobalamin.
Steroids may be tried in case of no response.
