I have a kid 10 years and we make the following investigations for him :
- Laboratory investigations for kidney functions and CBC
- Ultrasound
- VOIDING CYSTOURETHROGRAM

All inestigations are attached , wait for your advise on the following :
- Kidney status ( The effect of the hydrophones is severe on kidney tissue and nephrons and that is reversed or not ?
- What is the additional required investigations to diagnose and assess the case ?
- What is procedure advised and what is the complications ?

Your son needs ablation of valve.

Detailed Answer:

Hi,

Your son has an obstructing posterior urethral valve. This is blocking the urine from coming out the urinary bladder.

The blockage caused the increase in pressure in the kidneys and ureter causing them to be swollen and tortuous. This also makes the kidneys not to function well.

The operation that your son needs ASAP is posterior valve ablation. This is endoscopic removal of the blocking tissue. This is a straight forward procedure and has minimal complications if any at all. Procedure is done under anesthesia and usually takes less than an hour.

Whether the kidneys would Recover after the procedure only time can tell.

What is you opinion for his current kidney function based on the ultrasound and blood tests attached ? or they are not an accurate measure?

Do you assess his case as Chronic kidney failure ?

During the procedure : Do you think he will need "proximal diversion" as he has very thick bladder walls ?

As described in "VOIDING CYSTOURETHROGRAM" report VUR degree now is 5 , what is the effect of the procedure on that ?

GFR for kidney function. Ablation might all be needed.

Hi,

In my opinion, ablation might all be needed and assess his voiding after ablation if diversion might still be needed or not. I believe addressing the primary problem might fix all his problems in the urinary system.

GFR after fixing the valve can determine his kidney function and to determine if he has kidney failure or not. His creatinine is still normal which is good and it means that the kidney is still functioning well enough at this time.
