Question: Hello Doctor, My name is XXXXXXX and i live in chennai.4 weeks back i had severe cold.I met MBBS doctor near by after three days because of headache, little bit right ear and face pain.He suggested Augument 650 for 5 days. After that i still feel no major improvement. Then i consulted ENT in chennai and did CT scan and it says right maxillary sinusitis.Doctor says go for endoscopic surgery. Then i got little bit afraid and spoke to ENT doctor in my native. He said we will try with medicine for couple of days and suggested ceftom 250 tablets. After taking tablets for 7 days i feel better now but still i feel little bit right ear pain, neck pain and eyebrows pain(not unbearable but still some pain). When i spoke to ENT doctor in my native he suggested to use sefodex-350 for 4 days. My doubt are.. 1.can sinusitis be cured with medicines? 2.are all my neck pain, eye brows pain, ear pain and heaviness in head becasue of sinusits? 3.Is endoscopis operation is must?how simple it is?i hear patient can go home on same day?will be any soft tissues cut?and also i hear once operation is done it will be treated as problematic area forever.is it true? 4.How long i can postpone the surgery? Thanksin advance for all your help on this regards.

Brief Answer: hi, chronic sinusitis, endoscopic surgery. Detailed Answer: Hi XXXXXXX Yes sinusitis can be treated with medicines like antibiotics, decongestant, antihistamines and anti inflammatory drugs. And yes all your pains and heaviness of head is most likely due to sinusitis. Surgery is not a must for everyone. But if sinusitis does not reduce with conservative medicines , and if sinusitis is recurring very frequently, may be 3 episodes in 6 months then we advise for surgery. And if the sinusitis is associated with nasal poylp, nasal mass, fungal sinusitis then also surgery is a must. Surgery for sinusitis is a endoscopic surgery. In this surgery mainly we clear the sinuses and provide good ventilation for the sinuses. We do have to cut the soft tissue, debride, drain and then clear the sinuses. It's a day care procedure, patient can go home the same day. Once the surgery is done,, there are very less chances for sinusitis to recur. And yes the operated area is treated as problematic area until and otherwise you are undergoing second surgery. In my opinion if this is your first episode of sinusitis then you can wait and watch. If you are a chronic patient of sinusitis then I suggest you choose surgery. Hope I have answered all your questions. Let me know if you need clarifications. Regards
