Can Ovarian Fibroid Cause Infertility?

Question: Hello Dr, I am 36 yrs old, trying to get pregnant, from last 3 months on medication and injections HCG 5000. it has been detected a fibroid measuring 6.4x5.9x5.7 cm size is seen in close proximately to fundic-right lateral wall of the uterus, this is more likely subserosal uterine fibroid rather than broad ligament fibroid. I have been taking ovacare and my husband is on oligocare forte. My periods are regular . this time however during my period noticed the blood qty going very less and 3rd day it was just a spot. Is ther chnages of me being close to pregnancy. We are desperately waiting for this moment. Can you please suggest the next steps to get pregnant soon or is ther any other way. Hopingly waiting for your response. Thankyou.

Your age is 36 years and you need to have aggressive management of your infertility since after the age of 35 years, the quality and quantity of eggs go on diminishing and the chances of pregnancy too.
Currently you are on Ovacare and your husband is on Oligocare. Both these medicines are mineral compounds. The manufacturers claim that they increase the fertility; however, the prerequisits are that there should be no other abnormality. You may continue these medicines without any major harm.
Have you undergone any investigations work up for your infertility problem? If so, please upload the reports and images for my review. To start with, you will have to assess the following aspects:
1.     Whether you form the eggs regularly: The simplest way to find this out is to submit yourself for Ultrasonography ovulation monitoring. This will give the evidence of ovulation and even you will come to know when the eggs are being released. Accordingly you may plan your sexual relations to have better results. More details also are very informative. The size of follicle plays at the time of ovulation plays an important role in successful pregnancy. The eggs released from follicles of size between 18 mm and 21 mm size are healthiest and are more likely to result into pregnancy. Moreover, it informs about the endometrial thickness. Endometrium of thickness between 9 mm and 14 mm is most conducive for successful implantation of the fertilized ovum. You are being given Inj. hCG. This injection is given to induce ovulation in case of ovulation failure. Usually 4 to 24 hours after the injection the egg is released. Are following it with ovulation monitoring? Have you been assessed for ovulation and are given this injection because you do not ovulate naturally? You are having regular menstruation. This is a good sign. Usually such menstruations are ovulatory; though it is not a rule. Later on you may be advised some hormonal profile.
2.     You should find out that your reproductory system is patent and do not have any organic lesion in it. Hysterosalpingography, Hysteroscopy and Laparoscopy are the procedures which help to ascertain this. Though these are invasive procedures, they are very simple and informative. You are having a sizable fibroid near the right insertion of tube into the uterus. It is likely that the tube may be blocked at this site which will prevent the meeting of the sperms and the egg.
3.     Please get the semen examination of your husband done after 4 days of sexual abstinence. This will inform about the quality and quantity of the sperms in your husband’s semen.
The very fact you had menses after the unprotected sex suggests that you are not pregnant. The first cardinal sign of pregnancy is missing a period. However, occasionally, some bleeding may happen in very early stage of pregnancy. To confirm the status of pregnancy, please get your Beta hCG test done on the blood 10 days after the last unprotected sex. Values of hCG under 5 mIU/ml are found in non-pregnant state while values more than 25 mIU/ml are diagnostic of pregnancy.
