Can Myositis Affect Smooth And Skeletal Muscle Causing Pain?

Question: Im 55 yo male since age 43 have had cpks between 350-450. Prior to this they were 60-70. Since then anytime ive gone back to the gym and have done chest exercises especially weights i have intractable somatic and visceral pain. Had a cholecystectomy 9 yrs ago for biliary duskinesia and 2 yrs ago had ercp with sphincterotomy. My question is can the myositis affect smooth as well as skeletal muscle and when i get my next egd would it ne worthwhile to biopsy and stain areas of esoph stomach and duodenum to see if the myositis is the underlying cause of these severe intermittent bouts of pain occurring every 2 yrs for about 1-2 wks at a time

I m dralok, please to answer your health related query today. yes it can affect skeletal muscle as well as smooth muscle but it is not necessary that patient must go for biopsy if cpk, Ldh test are positive. Although biopsy of skeletal muscle is necessary to make a diagnosis if diagnosis is not clear. but staining of gastrointestinal system is not necessary. treatment is steroids and other symptomatic management
Myositis is usually painless condition, so if there is any pain in the gastrointestinal system, you need to be seen by a physician for proper evaluation

So ive been on ppi's and h2 blockers but when these attacks hit they only help minimally. So what im trying to tease out unless theyre comorbid is the acute albeit temporary pain of an exercise induced flareup of the myositis causing this gi pain or is it from a pancreaticobiliary origin. Btw lfts amylase lipase were NL all 3 times this has occurred. Here in the USA they treat the myositis with IVIG if prednisone fails. Have never tried it secondary to thinking origin of pain was GI. Thx i know this is a tough one thx again- XXXXXXX

If PPI fails, then you can try its combination with itopride and perinorm.

For myositis, methotrexate can help and also cyclophosphamide if lung or kidney also involved.
