There are few things that have to be considered in your case for the fatigue you are experiencing
-Hypothyroidism: Since you are on thyroid supplementation, inadequate supplementation can cause fatigue.
Even if you have been on the same dose for long periods of time, dose requirements can change from time to time. T3, T4 and Tsh evaluation can assess thyroid status.
Thyroid requirements can increase during the time.of illness like flu, sinusitis etc.
-Depression: Worsening Depression can present as fatigue. If that is the cause the dose of citalopram needs to be adjusted or switched to another antidepressant.
-Nonfunctioning ovaries: Since you are around menopausal age, your ovaries must have likely stopped producing estrogen which can contribute to fatigue. LH/FSH levels, serum estrogen levels will assess the function of ovaries.
Detailed physical and mental evaluation, Hb levels, complete metabolic profile, Thyroid function test, lh, FSH levels and estrogen levels are recommended at this point.
