Can Gestational Diabetes Affect The Development Of Testicles Of Fetus?

Question: Back in 1975 how good was blood sugar control of an insulin dependent lady. My mom had acquired type 1 diabetes mellitus. I heard that the high blood sugar environment of the uterus could harm the virilization of the foetus. Let me be clear; the testicles and penis form during 12 weeks into the pregnancy. I can only think the high blood sugar levels would decrease the testosterone level in the uterus and harm the masculinisation of the baby. I know later on the feats makes his own insulin but not at 12 weeks. Is there some trusty to what I think?

1. Development of diabetes during pregnancy is known as gestational diabetes mellitus(GDM). The management of GDM require only insulin. During early 1975 GDM was controlled with insulin, but the chances of reactions were more due to issues on purification and development?

This is different from type 1 and type 2 diabetes. The diabetes will develop only during pregnancy and may or may not continue post pregnancy.

2. Fetus will start producing insulin by 9th week of pregnancy.

3. During first trimester there is more stimulation of maternal pancreatic beta cells to compensate for raised sugar levels. So there is no much effect on fetus due to diabetes. But if the sugar level continued to raise or very high then the fetus will start producing more of its own insulin as insulin does not cross placenta to compensate for raised sugar level.

4. About side effects, there is no much side effect on the development of sexual organs or virilization. It mainly depends on development of sex hormones and not on insulin level.

5. The chance of congenital anamolies are there. But w.r.t development of sexual organs no such relationship has been established.

So in summary there was no much effect on sex organs due to GDM. But due to effect of insulin that is secreted by fetus, the somatic growth was more in fetus and hence large babu will be born.

she did not have gestational diabetes. She was taking insulin even before she got pregnant. When I was 5 my brother told me I had a small penis.
Is it possibly linked to my mom being diabetic before and during pregnancy. Also I am a couple inches shorter than my brother.

No. Diabetes is not linked to development of sexual characteristics. Infact in uncontrolled diabetes during pregnancy due to increased secretion of insulin from foetus, there will be macrosomia (more body weight and large baby).

Also the size of penis will be varing even in child hood. That will increase with age and there is no direct relationship has been established for variation in size of penis due to diabetes.

After birth the entire development (physical: height, weight) is based on your physical activities, nutrition status and also your health. These are modifiable factors that affect height.

So your height is not related to your mothers diabetic status during gestation. The only relation we can predict is, if any anamolies you develop at birth.

What is expected bio available testosterone level in a healthy male 38 years old; normal weight; physically active; non smoking.
I ask the same question about SHBG; Prolactin
also ; a question; what do you know about how kidney disorder effect childhood growth. If a child did not use public restrooms as a kid out of fear would this effect their calcitriol level and parhyroid hormone level to cause reduced height? Bascially from 5 to 12 years old I did not urinate at school daily from 7am to 3pm. Please comment.

Thanks for the query.

The normal testosterone level in healthy adult male is around 270-1070 ng/dl, peaks at 20 years and later declines and those of SHBG and prolactin are 20-60nmol/l and 4-12mcg/l respectively.

If renal problem at that age group then definately affect the height due to physical illness. Renal problem not only affect calcium metabolism but also can reduce Hb due to reduced secretion of erythropoietin. The calcitrion and parathyroid hormone can vary in renal disease as kidney is an integral part of calcium metabolism and hence we can definately see variation if any problem in kidney.

Not urinating in public rest room may not affect but if any renal problem then definately that will affect height. Not only kidney any chronic disease or even not physically active before puberty can affect the height.

The most important hormones that control height are thyroid and growth hormone. If any variations before puberty then there will be stunted or reduced height.

What is HB; what is erythropoietin. Also you said renal problem can effect height due to physical illness? I was not sick as a child. But for 7 years i did not go to the bathroom during the daytime. Well, at least for 5 of those years. I urinate when I woke up; then go to school and would withhold XXXXXXX to urinate until I got home. I always urinated before going to bed. Could the daytime not urinating cause chronic renal insufficiency or an effect on the bone growth and metabolism? The question is that if you do not urinate for 8 hours per day ; during school day; will this cause a renal insufficiency that would effect the calcitriol level and parathyroide hormone? Would this reduce height growth?

Hi

1. Hb means hemoglobin. This is required to transport oxygen to the tissue and erythropoietin is secreted by kidney and is required to stimulate red blood cells production. If any of these is reduced then anemia will result and hence the tissue will receive less oxygen supply.

2. No, Not peeing for 8 hours will not cause chronic renal problem. This will lead to increased bladder capacity and once the capacity is over then you can't control more and due to pain and normal nervous system activity you will urinate. THis wont affect your growth. Usually if any problem to kidney, there will be lot of clinical features like edema around eyes and later whole body, severe head ache, uneaziness etc. So since you were healthy during childhood days none of the health related problems has affected your growth.

3. Variation of 1-2 inches of height among siblings is a common feature. This may be due to genetic factors or nutritional status or even physical activities till the closure of epiphysis in bones. So there need not be any health related issues. If growth is stunted significantly when compared to all family members then only there may be chances of any health related issues.

Dear Dr.

A question; may I ask?
Are you taller than your dad? If so; are your hands larger than your dad's; do you know? I believe that height is related to calcium AND protein intake because long bones require protein to make the muscle that relates to them. Hands and foot growth are less protein dependent but more indicative of growth hormone and calcium metabolism. Now if I may say; i do not find it normal that i have smaller hands than my dad unless i had a calcium metabolism deficiency.
I am inclined to think that without the deficiency i would have been quite a bit taller.
For example my hand length is only 19cm; compared to my dad which is 19.5cm. Also his feet are size 9.5; mine are 9. My brother are size 10 and he is 5 9.

Concerning your point about kidney disease; yes i understand what you are saying. But studies have shown that even when a bladder is 60% full renal function is decreased (lower GFR). Of course, if I had to urinate because it was full to the point of you can no longer hold it ; i would urinate. Also even mild chronic renal insufficiency is known to cause growth retardation. And without other symptoms typical of kidney disease. It seems that growth retardation can be the first sign and asymptomatic of normal kidney disease. As in stage 1 or 2 kidney disease. PLEASE COMMENT

So you know my parents had considered putting me on growth hormone but the doctor advised against it because he felt i would not be shorter than 95% of other men. PLEASE COMMENT

i do wish however he had done some simple tests to see if there was any other reason why I was growing slowly.

1. For me my and my father's height are same.

2. Decreased GFR does not mean it affect calcium metabolism. All these (excretory function, calcium metabolism, Vit D, erythropoietin secretion, maintaining blood pressure etc) are different function of kidney. If GFR is reduced that doesn't mean all functions are reduced and reduction of GFR when bladder is full is a compensatory mechanism. This doesn't mean renal failure.

3. There is no significant variation in height that means it may be due to your genetic makeup or nutrition or due to physical activity.

4. At your age (38) growth hormone supplements will not help in growing taller but to increase size. So even growth hormone supplementation is of no use. If you want to grow taller than surgical mode is the only way as the epiphysis in bones would have been already fused.

5. Yes during your teen age if any variations in calcium level will affect growing taller but you don't had any features suggestive of eitger renal problem or decreased calcium. So it's very difficult to find out the cause if any.

You are the same height as your dad?
Can I ask please how tall was your mom?
You know you are taller than the average XXXXXXX This is not the case for me as I live in the West. Imagine if you were 5 foot 2; then you would understand a bit how I feel. Average 20 year old Americans are 5 10.5. I am five 7. So I have a bit of a handicap in sport and in dating. Many USA women will not date guys shorter than 5 10. 5 9 is even short. I know in XXXXXXX it is very different and height is not so important. This is obvious otherwise my parents would have done more than take me once to a general doctor to see if I should get growth hormone. When he said no; that was it. No extra tests; nothing done to see why I was short.

I have looked into surgical height methods; which ones are the best? Least painful with least side effects. I am no coward to surgery as I already broke both my femurs during a paragliding accident 10 years ago.

I think I was average height age 3 to 6 or so. But my parents never kept any growth charts so I would not know. When I went to school aged 5 or so ; I stopped urinating normally during the day at school. This started because once at a public bathroom my brother told me I had a small penis.

They use GFR to show kidney function. Studies have shown that with an even minor reduction in GFR growth can be harmed. And studies have shown that a full bladder (even 60% reduces growth).
Would it be of use to test my calcitriol or parathyroide hormone. If harm had been done it is likely that you would see altered levels perhaps even now? And for the ecographie of kidneys what to look for? Dialated urethers?

Interesting to know I gained weight becoming chubby starting around age 9; this would correspond to a reduced kidney function? I kind of feel that if you don't urinate for 8 hours each day you are reducing your metabolism. And clearly the bone metabolism as kidney effect this. It is like I stopped growing up so I became chubby. It is around 10 when they noticed I Was short. They were concerned. Soon i went to a different school with toilets i could use more regularly. By age 12 I tall enough that I was not in the bottom 5% so they were less concerned.

I had my sperm count checked in 2012 and it was below average at 50 million/ML. Total was around 180 million.
My bio available testosterone was around 2.24 nmol/L. Should it not be something like 4 or so? My total Testosterone was around 15 or 440 (using US units). Even this should be quite a bit higher. My SHBG was around 50. My prolactin was 17 once; and 20 more recently. All these point to an androgen deficiency which i have probably had my whole life. And certainly during puberty. My mom had Type I diabetes; which she acquired while living in India; a virus attached her pancreas. She was already taking insulin ; one year before I Was born in 1975. Does this have an effect on fetal gonad development (as in testicles and penis); and would this cause me to perhaps grow to be less tall?

Would growth hormone have any impact on my hands or feet or size of larynx ; as these have less growth plates then femurs? What if I supplemented this with androgenic testosterone and HCG (perhaps 2000IU 3 times a week)? And an aromatese inhibitor such as arimadex. I think my voice also suffered some and is not deep as it should be. Do you have an idea of the type of testosterone that I would take; the androgenic one (not anabolic). I just was never exposed to as much testosterone as I should have been. I read stories about guys even 26 who took testosterone and it helped them a lot.

With so many queries, I like to tell about surgical method first.

1. It called as Ilizarov technique: Long bone is surgically fractured and the ring apparatus is attached and as the patient recovers, the fractured bone begins to grow togetherand during that time the frame is adjusted thus increasing the space between two rings. As the rings are connected to opposite sides of the fracture, this adjustment, done four times a day, moves the now-healing fracture apart by approximately one millimeter per day. Thus the height can be increased.

2. You can get done USG abdomen and pelvis or even MRI scan to look for the status of kidney and also check for creatinine clearance and Serum creatinine levels to check the functional status of kidney. The current status does not tell about the past.

3. Growing chubby tells about the low physical activity. If any hormonal problem like hypothyroidism then also you develop cretinism(chubby) and short stature, but no other symptoms are towards it.

4. About androgen defeciency, if the testosterone level are reduced during puberty then the secondary sexual characteristics will be affected and even growth. But we can't comment on it which was 20 years before.
If your voice is not deep as male then the daily voice training/exercise by pressing the thyroid cartilage little deeper will help.

If you take any drugs, whether growth hormone or steroids or testosterone, there won't be any effect on growth as the epiphysis is already been fused. At your age only surgical procedure can help.

Doing stretching exercise can help in increasing height by 1/2 - 1 inch due to relaxation of all joints.

About type 1 diabets on insulin affecting height to her kid, no studies has been done an corelation has been established. So difficult to comment whether this has affected you.
