Can Bilateral Varicocelectomy Be Operated From Different Sites?

default
Posted on Wed, 16 Dec 2015
Question: pls i just did a bilateral varicocelectomy,to my XXXXXXX when i read it online i learned they will make a small cut at the scrotum then do the surgery from there, but my urologist, just made 2 small cut below my abdomen, both left and right side, i went home 2 hrs after the surgery completed, my problem is i still have the same feeling i had before going for the surgery which having small pain in my left testis sometimes on the right which the ultrasound came out that it was varicocele, now a day after the surgery i noticed my scrotal sack, has now become bigger , like it has expanded, my questions are :

1 ,could it be cos i just had the surgery and the scrotum is getting healthier cos it was like shrink stuff before or is there a cause for alarm?
,2,why am i havin this same feeling around my testis again, especially on the left side though the pain is not much as it was before the surgery,
3,why was the approach of the surgery different to what read online, why made the cut on both side of my below abdomen instead of the scrotum.
4,thus the sperm increase automatically without using any sperm booster ,if there is any?

Brief Answer:
Varicocele can be operated from different sites .

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you have been operated for bilateral Varicocelectomy and surgeon has taken two incisions on lower abdomen instead of through scrotal incisions .

The primary object in surgery for Varicocelectomy is to ligate some of the dilated tortuous testicular veins .There are different approaches to do this surgery .
1) Intra Peritoneal as done in Laproscopic Varicocelectomy
2) Through lower abdominal incisions
3) Scrotal incision

It depends upon the operating surgeon ,his experience and the way he does it routinely .It has little significance as far as outcome and results of the surgery are concerned .

As regards your queries I would answer them point to point .
1) Swelling of the scrotal sac is natural ans normal after surgery and is due to handling and dissection of spermatic cord structures .This subsides within a week or two.
2) Discomfort around the testis takes time to go away and takes two to three weeks as the healing progresses .
3)It is not a different approach but one of the approach to perform surgery .
4)It is very difficult to predict as regards increase in sperm count after Varicocelectomy .There are 30% chances of improving the sperm count .

Hope I have answered your query ,please feel free to ask if you have more questions ,I shall be happy to help you
Thanks and Regards.

Brief Answer:
Normally we advise abstinence for a month afrer surgery .

Detailed Answer:
Hello
Thanks for follow up .

Normally we advise to refrain from sexual activities either sexual intercourse or masturbation for one month till it heals up completely .

Of course it is not the rule and depends upon the individual tolerance and pain threshold .

Thanks and Regards.

Detailed Answer:
Hello
Thanks for follow up .If you are comfortable you can have sex .

As regards sperm booster there is no medicine that cab boost up sperm production .All the medications available in market are prescribed without having any scientific data accepted by WHO to prove that they really boost up sperm production .

You can take Adyzoa prescribed by your Physician ,

Brief Answer:
You need to consult Urologist to confirm about length of our penis .

Detailed Answer:
Thanks for follow up.

You need to consult qualified Urologist ti confirm whether you really have small penis .Do not judge it after what you read on internet .

As such length of the penis in flaccid sate has no significance at all .It is hardness of the penis that is important for sexual satisfaction of both the partners .

You can upload photograph of your penis ,

Thanks and Regards.
