Posted on Wed, 2 Nov 2016
Question: My mom had a rituxan dose for CLL on THursday. We are now in the hospital on neutropenic precautions with pneumonia. Her WBC went for 15.7 to 1.2 and today is up to 4.1 on neupogen. She has pneumonia and is on cefapime and azithromycin IV. She is very tired today and sleeping all day. Her H&H is 10.4 and platelets 45. What is causing her to be so tired?

Brief Answer:
Low hemoglobin and side effects of medications

Detailed Answer:
Dear Ma'am, Hi & Welcome.
Thanks for posting your query on healthcare magic.
I appreciate your concern for your beloved mother.
Her low hemoglobin seems to be responsible for the tiredness she is suffering from. Besides, other medications received by her can also cause such side effects.
Hope to have answered your query.
Best wishes.

Sorry but that is not the answer. Her repeat Hbg on Friday was 11.6. She is tired and I believe the rituxan takes its toll. Do you know anything about the side effects if monoclonal antibody treatment.
Every time she has one of those treatments she takes weeks to build herself back up.

My mother will be 99 on Thursday. OUtside of CLL and short term memory loss, she does pretty well. Her primary can only try to get me to sign a DNR paper. If it weren't for me they would have killed her by now. Medicine is backward. Just because she is 99 they don't believe she can be well, active, and have a quality of life. Life is sacred and I love my mom. She looks great and one of her doctors said she looks like she is in her 70's. 99 is the new 79. What is so difficult about that? I'm taking my mom to XXXXXXX as soon as she is stronger so we can shop, go to the beach, and enjoy life a little. Most people would put her in assisted living or a nursing home by now. I am a nurse practitioner and believe most people in the medical profession are stupid in general. That includes her oncologist and family medicine doctor.

Dear XXXXXXX Hi & Welcome back.
Thanks for your reply.
I really appreciate your positive feelings & attitude towards your beloved mother. Glad to know that she will be 99 years in a couple of days, she will live longer & healthy In XXXXXXX Allah. It's unfortunate to know about the unethical attitude of some of the medical professionals.

Common side effects of Rituximab include:
1. Infusion related reactions like headache, fever, chills, flushing, night sweats, fall in BP, stomach pain, weakness, muscle or joint pain.
2. Delayed side effects like increased risk of infection, tumor lysis syndrome, swelling of the limbs, difficulty in breathing, vomiting, loss of appetite, etc.

Wishing your mother a speedy recovery.
Take care.
