I have studied your question and I can assure you that none of these condition will cause any problem in speculum examination.

But before I advised you any further I would like to know little details about your conditions.
1. What type of speculum examination you are planning to go for?
2. Are you taking any medications?
3. What is the severity of pain(rating it from 0 to 10, where 0 is least and 10 is max) or any swelling at joints currently?

Please reply back. I will try my best to help you.

I also find that I can not, due to the pain, use a small tampon when my period is heavy. I feel as though I will pass out with the pain. Sexual penetration makes me feel as though I will pass out. The pain is 9/10 for penetration with a speculum, tampon or a penis. I may have to have further investigation to find out why I get frequent ecoli infections in urinary tract, vagina and back passage

Brief Answer:
This can be due to STD

Detailed Answer:
Hello,
I think you might be having STD. Because of this even slightest penetration causes you severe pain. I would recommend you to get a culture of vaginal swab.
This can cause burning urination and back pain also.
thanks
