Question: Is Syntocinon the drug that can have an off-label use with a local anesthetic in patients with hypertension in whom one would prefer not to use epinephrine with the local anesthetic? The procedure under consideration is colpectomy and colpocleisis. Please refer my question to a urogynecologiest.

Syntocinon contains hormone Oxytocin and causes a smooth muscle contraction and mainly causes uterine contractions and helps in induction of labor pains. It is not used with a local anaesthetic to decrease the bleeding in place of epinephrine routinely.

It can be safely used in hypertensive patients with no cardiac side effects. It can be used off label with a local anaesthetic for short procedures. It depends on your anaesthetist.

Dear Dr. Tayal,
Thank you for your response to my question regarding the off-label use of Syntocinon with a local anesthetic to help with hemostasis for a colpectomy and colpocleisis in a hypertensive patient. Do you have an opinion on the amount/concentration to be used with lidocaine or bupivacaine hydrochloride and on the maximum dose that should be used?

It is an off label use and the dosage may vary from patient to patient. Usually started with 5 IU and the dosage is then adjusted according to the need.
