Question: I'm using Restasis right now and just started but want to know if i can use a lubricant eye drop with restasis as well and if i can which lubricant eye drop do you recommend i take?



I read an article on the Internet that REFRESH OPTIVE®Advanced Preservative-Free Lubricant Eye Drops works on all three layers of your tear film for Dry Eye symptom relief is this true?

Does it do all of this?

Lubricates for moisturizing Dry Eye relief
Hydrates the eye surface cells
Protects your natural tears from evaporating with a lipid enhancement


Another question do you recommend taking the Refresh Optive Preservative-Free or no ?

What's the different between Preservative-Free and Preservative?

I usually recommend and prefer to use preservative free lubricant eye drops like Refresh tears or Refresh optive.
Lubricant eye drops does not interfere with the Restasis but it supplements and helps the treatment process.
Lubricant eye drops mainly help in providing the aqueous layer of the tear film to the eye. Aqueous layer accounts for the majority of the tear film and hence lubricants helps in most of the dry eyes.
Restasis will act by reducing the inflammation of the ocular surface caused by dry eye. It helps in retaining the tear film by repairing the ocular surface damage caused by dry eye. Hence it requires 4-6 weeks to show symptomatic improvement. It needs to be containued for 3-6 months or more depending on the severity for symptomatic improvement.

Another question do you recommend me to put refresh optive advanced lubricant eye drops after or before putting the restasis eye drops?

How long in between eye drops do i have to wait ?

Another question when inserting any eye drop whats the proper way to inserting a eye drop into your eye. Which way do you recommend the vest way possible to insert an eye drop into your eye?



Another thing is that both of my eyes are having eyelid twitches on the bottom eyelid of my eyes. Whats the reason for this?

Restasis is one of the effective drug available for treatment of dry eye. It targets the basic pathology of dry eye and treats it.

Lubricating eye drops are preferably used after restasis with a gap of 10 minutes.

The best way to instill eye drops is by pulling the eyelid down and instilling one drop and closing the eyelid for 2-3 minutes.

The twitching is probably because of dry eyes and should resolve with treatment of dry eyes.

So you want me to instill both eye drops the same way by pulling my lower eyelid forming a pocket and instill the eye drop inside the pocket instead of instilling the eye drop directly in the center of my eye ?

Yes you need to close the eyes after instilling refresh tears. All the eye drops need to be used in this method as it increases the efficacy as they are retained for a longer time.

Refresh tears can be used every hourly also depending on the severity. As of now you can use it 4 times a day and if the symptoms persist you can use it more frequently.

Restasis comes in single dose vials and you need to use and discard daily. Refresh tears eye drops can be used till it gets over or for one month after it is opened.

my eye doctor told me that i can use 1 vial of Restasis 2 times a day rather then 2 vials per day as long as i put the top back on and stand the vial up withoit touching the tip to anything and leave it standing up.

My eye doctor gave me a brief answer saying that Restasis attacks T cells that are trying to go to the Lacrimal glands that are inflammed which causes dryness to the ocular surface of the eye and causes inflammation. He also mentioned that these T cells can secrete T lymphocytes which i dont know what that is at all


What are T cells ?

Can Restasis fully treat T cells and eliminate them completely?

What exactly are T lymphocytes?

My eye doctor also mentioned Goblet cells?

What are goblet cells and can restasis eliminate these goblet cells?

Restasis is the name for pharmacological content cyclosporin. Cyclosporin is a immunomodulator and immunosuppressive agent.

The initial treatment of dry eyes was by tear supplements and by reducing the tear evaporation.
However the recent understanding of the dry eye has shown that it is basically because of inflammation of the ocular surface called as keratoconjunctivitis sicca. This has led to the use of antiinflammatory drugs like cyclosporine. The treatment has proven to be very effective as compared to the traditional treatment of use of artificial tears. Infact combining both as in your case will provide excellent results.

Restasis acts by inhibiting the inflammation induced by the T cells. T cells are also called as T lymphocytes and they take part in the cell mediated immunity causing inflammation which will damage the lacrimal gland which secretes the tears ultimately resulting in damage to the ocular surface resulting in dry eye.
Restasis binds to the T cells inhibiting their action and prevents the damage to the lacrimal gland.

Goblet cells are normal conjunctival glands which secrete the mucin layer of the tear film. Inflammation will damage these cells. The anti-inflammatory activity of the T cells will lead to increase in the number of goblet cells and increase the mucin secretions.

Thus Restasis acts by targeting the basic pathology which is inflammation of the lacrimal gland and the ocular surface, thus it is superior to the use of artificial tears alone.

But please do remember dry eye is a chronic condition and treatment is required for 3-6 months and some time even more for complete relief.

I want you to evaluate my story on how i got my dry eyes and i want you to let me know if im taking the absolute right approach and best possible treatment plan to get complete relief of my chronic dry eye condition


So I wore colored contact lenses for several years. I say about 10 years and i used to get infections and irritations on my eyes and then evenutally one day i would put my contact lenses in my eye and my eyes would feel very dry and uncomfortable and i would day to myself what is going on. I would get a burning sensation and heating sensation and i would take them off but when taking them off i would get these severe itching sensations in my eye that would make me rub my eyes constantly because they would itch a lot after i take the contact lenses at day before going to sleep because i would keep my contact lenses for the majority of the day so evenutally i told my parents that i would go ahead and get LASIK surgery done that way i wouldnt have to rely on my contact lenses. So i got the LASIK surgery done on December of 2013 and its been already 9 months already since i had the LASIK procedure done but even before having LASIK surgery i already had dry eyes because of the situation of the contact lenses.

So by reading my story on how i developed my dry eyes can you evaluate and tell me if im taking the best approach and the best possible treatment for my dry eyes ?

I reviewed your history. Because of long term use of contact lens you had contact lens induced allergic conjunctivitis and dry eyes.
This dry eye is probably exaggerated by the Lasik surgery. Lasik surgery will normally lead to some amount of dryness of eye and this usually reverses by 3-6 months after Lasik surgery. But since you had pre existing dry eye it is taking longer time to respond.

The treatment by the use of restasis and refresh tears eye drops is in right direction and probably the most appropriate and best one for your condition. Hence you need to continue it for atleast 3-6 months before expecting improvement. If this does not respond next you can try lacrimal punctal plugs as an additional procedure.

Dry eyes will not usually present with redness. They present with irritation, grittiness, eye strain, blurring of vision....
The diagnosis is usually made by symptoms and simple tests like schirmers test, examination of ocular surface after staining with fluorescein or other similar stains.

Redness is usually a feature of acute inflammation and dry eye being a chronic condition will not present with redness and other acute symptoms.

I know i asked this question before but i have the Restasis phamlet and it clearly states that restasis doesnt contain any preservatives and that only 1 vial should be used each time and immediately discarded after each use. If thats the case then why did my eye doctor recommend me and told me that i can use 1 vial 2 times a day and then discard it?

As I had told strictly speaking the Restasis eye drops of single vial use needs to be used for single use. But with precautions has your doctor has mentioned like as long as you put the top back on and stand the vial up without touching the tip to anything and leave it standing up and by avoiding contamination, it can be used for one day.

As I had mentioned in the previous answer it should not be used for the next day that is after 24 hours of opening the vial.

It is basically done to prevent contamination and degradation of the drug which occurs as it does not contain preservatives. Hence with care and precaution it has to be used while using the same vial for the second time in the same day. This is not at all advised to use the same vial after 24 hours of opening the vial.

If you are doubtful about the precautions then it is always better to use one vial for one use only and discard ot after every use.

Ok thank you another thing i forgot to mention is that 1 month ago i was rubbing my left eye which isnt dry at all and was perfectly fine but when i rubbed it enough it started fewling weird and then a couple of days later i got a red bump on the lower eyelid of my left eye and while i had that red bump on my eye i felt pain everytime i blinked and felt the sharp pain on the outer part of my left eye where the red bump was and now this is causing my upper eyelid to twitch. What is the cause of this? What do i have by evalutating what i just said because i dont feel dryness on my left eye but i do feel a weird sensation on the outer part of my left eye where i had the red bump on and a little dry on that outer part of my eye as well

Our tear film has three layers, outermost lipid layer which prevents evaporation of tears, middle aqueous layer which moistensthe eye surface, innermost mucin layer which anchors the tear film to the eye surface.

The commonest cause for dry eye is aqueous deficient dry eye which is caused by decreased tear secretions commonly because of decreased reflex stimulation as occurring in decreased corneal sensation following surgeries on cornea, refractive surgeries, diseases of lacrinal glands...
It is diagnosed by clinical examination and tests like schirmers tests.
It is treated by tear substitutes and antiinflammatory drugs like cyclosporine.

Lipid deficiency dry eye is because of decreased secretion from meibomisn glands which secrete lipids. The causes are meibominitis. The treatment is by treatment of meibominitis. It is identified slit lamp examination.

Mucin deficient dry eye is seen in damage to goblet cells of conjunctiva as in chemical or radiation injuries, XXXXXXX XXXXXXX syndrome, vitamin A deficiency. It is identified by conjunctival impression cytology. The treatment is by treating the underlying cause.

In your case it is aqueous deficiency dry eye and treatment is in right direction.

Oh ok perfect thank you for all of that information. In my case like i told you before with my story that led me to have Aqueous deficiency dry eye is that i wore color contact lenses for years that were perscribed obviously for my vision because i was near-sighted and i wore them for about 10 years before deciding to have LASIK surgery this past december of 2013 so even before having my LASIK surgery i had dry eyes. So what exactly happened in my scenario that made me get aqueous dry eye?
Was it because like you mentioned decreased tear secretions due to my contact lenses on my cornea and decreased reflex stimulation as occurring in decreased corneal sensation because you didnt mention that contact lenses could be a cause of aqueous dry eye.

So I've been on Restasis for 1 month now but i haven't noticed any improvement yet is that normal?


Another question I have is how long can it take before i actually notice an improvement?

When i first started the restasis treatment i would get side effects like a slight burning in the eye and also like something was in my eye but now everytime i put a restasis eye drop i dont have any side effects i dont know why?

Restasis is known to cause side effects like stinging sensation, redness, blurred vision....
Most of these are transient and resolve by 2-4 weeks. As stinging sensation has resolved in your case, blurred vision should resolve.

I woke up this morning with my right eye feeling full of heat and throbbing... Like the feeling it is irritated and this is the eye thats getting treated with Restasis. Even when i close my eyes you can feel that my right eye is full of heat like the feeling i have red eyes but i dont. Whats going on? Why am I feeling this sensation on my right eye that im using restasis on and this is the first time i wake up like this.

I reviewed your history. The sensations of irritation, throbbing.... probably indicate side effect of Restasis. Use the artificial tear eye drop once or twice once in a hour. I was rubbing my left eye when i felt a weird sensation after i stop rubbing my left eye and then a few days later i got a red bump in the inside of my lower eyelid which was red and was located at the outer part of my left eye which is where i felt sharp pain everytime i blinked and now my left eye feels weird and at times it gets a little blurry when i read or do anything thats not reading. Im freaking out. I dont know what this is?

Lets say for example that i didn't put in the first drop of restasis correctly and feel i didnt get the most of it can i put another drop of restasis in my eye o will it cause harm ?

If we instill more than one drop it will overflow.
Hence if you do not instill the first drop properly and if you want to put one more drop wipe it and you can instill the eye drop of restasis immediately following wiping it.
