Can HIV Be Contracted Through Anal Sex?

default
Posted on Tue, 10 Jan 2017
Question: If I had received anal sex with a hiv positive partner while I'm on PEP from a recent possible exposure, approximately 10 days before but from penetrative anal sex; what are my (worst case scenario) chances of contracting hiv ???
I have never done this before and never will, I questioned my sexuality and met people from the internet, I hate myself for it and I know 100% it wasn't right for me and I am straight.. please help I'm sorry for the long question, I'm really scared. This is the stupidest thing I have ever done

Brief Answer:
HIV risk is considerably reduced by PEP.

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
Recipient anal sex has the highest transmission risk for HIV among all types of sexual activities as per CDC, XXXXXXX That too, penetration by a known HIV positive individual carries a high risk for HIV.

But this risk is very much reduced if the viral load of that HIV positive individual is undetectable (when he is on HAART).
Moreover as you are on PEP ( it amounts to PrEP). The chances of HIV infection is quiet unlikely. Nothing to worry. As you are on PEP the regular check up and follow up for 6 months is necessary as the guidelines of PEP suggests.

Thank you, do I need to go back to the sexual health clinic and tell them about this second time? I was only given a 28 day supply of PEP and the follow up was just blood tests, is this all ok or should I go back to the clinic and tell them everything? Thank you for responding so fast I am terrified.

Brief Answer:
Complete a course of PEP after this exposure

Detailed Answer:
Hi,
Welcome back.
It is better to explain the fact to the treating doctor. It will be safer for you to extend the duration of PEP up to 28 days after the present exposure and have a follow up for 6 months since this exposure.

Thanks again,
Ok this its my final question; if you had to estimate a ballpark figure of the worst case scenario odds of me contracting HIV as it stands now, what would it be? (Could be between two different fractions)
Also, are there any other/extra things I can do to ensure I stay healthy? (Besides practicing safe sex) I have just started to get a cough and I'm still getting scared. I gave myself an enema with water but it was a day later and again a the next day with lightly soapy water, will this have helped?(I have never done this before either)
I know this is so ridiculous and self inflicted.
The rough calculation is very important to me because I'm so anxious, please help and thank you again.
Sincerely XXXXXXX XXXXXXX

Brief Answer:
Your symptoms are not related to HIV.

Detailed Answer:
Hi,
Welcome back.
Medicine is not mathematics to get constant answer.
30-40% of Spouses of HIV positive people remain HIV free in spite of regular sex contact for years together. They are called as sero-discordant couple. At the same time we knew people got the infection just with a single contact.
But I had not come across or heard from my colleagues a single case of PEP failure in getting HIV infection during my HIV practice of 27 years. When that individual's viral load is undetectable, then also the chances of getting HIV is remote. Safe sex is always better.
Washing with water and enema would not be of much use in preventing the infection. Earliest HIV manifestation can occur only after 3-4 weeks after the exposure in the presence of an infection, not earlier than this. So your cough is not related to HIV.
