Brief Answer:
Look for pain and palliative care specialist

Detailed Answer:
Hello,

Dilaudid is controlled substance and is under direct supervision of DEA. So doctors do not dispense these drugs without authentic reasons. If doctors prescribe these drugs over their limits they would surely be questioned.

Of all the specialists Pain and palliative care doctors have higher limits of prescribing hydromorphone, so you have to approach a pain medicine doctor and give proper symptoms and signs of pain to convince.

Search for the data in your area and call them up before you take the appointment.

Hope this is some useful to you.
