Dear Doctor
Please forgive me for takie ng your valuable time. I am in a confused state and in dialema. To do or not to do is my problem. Kindly go through the following with patience and advise me .
I am 75 year old male.
I had IHD since 1977. I had T wave inversion in V4,V5 and V6. I have under gone 2 D echo and TMT last year and both have come negative and normal . I had mild hypertension since 1981.Further i have Type II diabetes and BPH since 5 to 6 years,besides Hypothyroidsm.All are well under control and taking Betacard,Amtas,Glycomet,Deplatt, Urimax and Atorva.

I had BPPV since 2012.Ocassionally i experience it. I take vertin and stugeron.
I have taken ASA 50 from 1977 to 2014 and then ecosprin av 75 from 2014 to 2017. Then Deplatt cv stopping ecosprin av 75 from 2017.

Recently my cardioloigist left to USA for good. So when i am experiencing dizziness quite often and vertigo also sometimes, i approached a well known eminent cardiologist. He has asked me to discontinue ecosprin and also Ismo which tablet i am taking since 22 years, and said these two tablets cause vertigo and dizzziness.

When i stopped the dosage of Ismo 10 mg ,with the very first dose i am relieved of dizziness and vertigo. I have stopped ecosprin also since a month.

Now the problem comes here.I am afraid to stop Ecosprin altogether as it keeps my blood thin and helps in smooth flow in the veins and arteries. Since i have been taking Asprin for 40 years, I am not dared to stop it. This i cannot discuss with the cardio as I am afraid to talk to him.He may think that i have no confidence in him. I am taking Deplatt plain. Kindly advise whether Deplatt is enough or do i have to take ecosprin also. Please think deeply andadvise and save me from this dialema for which i shall be greatful to you sir. In case you requirre any further info i shall apprise.

Hello Sir

After carefully going through the detailed medical history provided by you I can very well understand your concern for your health and I would like to tell you that having normal echocardiogram and TMT results is a good prognostic indicator.

Ismo contains isosprbide mononitrate which in many individuals causes dizziness or headaches due to its vasodilator effect of blood vessels.

Aspirin on the other hand usually causes gastritis in many patients, vertigo and dizziness is not very common.

However if there is not stent in place there is no need of dual antiplatelets like aspirin and clopidogrel. Usually only aspirin is prescribed and in those cases where aspirin is not tolerated well patients can be safely switched to clopidogrel which is better tolerated in majority.

So yes clopidogrel alone is equally effective and can be safely continued.

Dear Doctor,
I profusely thank you for your nice reply.I dont have stents. Infact i have never experienced chest pain or sweating or angina so far in my life. I myself went to my cardilogist friend in 1977 and insisted for an ecg.Then only he found some disorder.
Dear Doctor,You have relieved me from my tension. Thank you sir.Now with confidence i can go with deplatt.Thank you once again
