The bright red spots are appearing secondary to trauma of intercourse or mastrubation. The trauma is much less when condom is used and hence lesser in number.

Will that area heal or is it expected to stay there for a while? It's been almost 5 months and hasn't gone away.

It is expected to stay like that. Everytime you have intercourse or mastrubate the area gets traumatized and hence it shall stay like that.
