Brief Answer:
NO SYMPTOMS, HSG CAN CONFIRM.

Detailed Answer:
Hello
Thanks for writing to us with your health concern.
There are no symptoms that you would feel regarding a failed tubal ligation, apart from pregnancy !
3 months after a tubal ligation, a procedure called HSG ( hysterosalpingogram ) should be done.
This is a kind of X ray where XXXXXXX is injected through the cervix, and the uterus, tubes are highlighted.
A tubal ligation shows up on this X ray.
If you have not conceived inspite of having had unprotected sex for the past 5 years, in itself that provides proof of the tubal having worked.
For objective evidence, just get the HSG done.
All the best
