Detailed Answer:
Hello!

Welcome on HCM!

Regarding your concern, I would like to explain that pacemaker implantation has recommended therapeutic indications irrispective of age.

Nevertheless, it is preferable after body growth process is finished, so no interference with pacemaker lead extension occurs.

Also, the medical indication for pacemaker implantation should be carefully reviewed.

Hope to have been helpful to you!

In case of any further uncertainties feel free to ask me again.

Kind regards,

Detailed Answer:
Hello again!

I would explain that even if a pacemaker is needed it is temporary and can be removed after the improvement of the situation.

Regarding medication, when there is overdose of benzodiazepines (which are the typical sleeping pills), Flumazenil can be used as an antidote, although it should be used with a lot of caution because of several adverse effects (it can even exacerbate the situation). That is why it is often not used in acute intoxications.

The main therapy in such cases are endovenous fluids with diuretics, which help increase the kidney filtration and excretion of the drug with urine (forced diuresis). Active carbon can be used after gastric lavage in order to avoid further intestinal absorbtion.

Other drugs which can be used are inotropes (adrenaline, dobutamine, etc.), which are used as a supportive therapy in case of low blood pressure and bradycardia.

If all these therapies are not efficient, temporary pacemaker is recommended.

Hope you will find this answer helpful!

Best wishes,

Dr. Iliri
